Key Responsibilities:

Personalized Care Planning: Collaborate with interdisciplinary teams to design and implement tailored treatment plans that address unique client needs and goals.

Wellness Program Development: Create and launch initiatives that promote healthy habits, disease prevention, and overall well-being for clients.

Health Education: Lead interactive groups and workshops that empower clients to manage complex medical conditions, make informed decisions, and adopt healthy lifestyles.

Care Coordination: Oversee hospital admissions and discharges, ensuring seamless transitions and continuity of care for Healthcare Home patients.

Patient Empowerment: Educate clients on their medical diagnoses, treatment options, and medication management, enabling them to take an active role in their healthcare journey.

Technology Integration: Implement and leverage innovative technology programs, such as CyberAccess and metabolic screening, to enhance client care and outcomes.

Performance Monitoring: Track, analyze, and report key performance measures, using data-driven insights to inform care decisions and drive quality improvement.

Effective Communication: Demonstrate exceptional verbal and written communication skills, actively listening to clients, and communicating complex information in a clear, concise manner.

Critical Thinking: Apply analytical skills and nursing expertise to make informed decisions, solve problems, and drive positive outcomes in client care.

Requirements:

Active RN or LPN license

Direct patient care experience preferred

Strong understanding of chronic disease processes

Excellent communication and interpersonal skills

Ability to adapt to new technologies

Critical thinking and problem-solving skills

Passion for exceptional patient care
