We are seeking a Mental Health Professional to join our team in our Assertive Community Treatment (ACT) Team in Canonsburg, PA and Waynesburg, PA. 
Earn $26.28/hour 
Position Details 
Schedule: Monday-Friday, 8 am-4 pm with one day 10 am-6 pm, with rotating on call weekends (8 am-4 pm) and holidays 
Duties and Responsibilities: 
As a member of the ACT Team, the Mental Health Professional possesses the clinical skills and experience necessary to assess, plan, develop, coordinate, and provide treatment within Washington and Greene Counties. This professional offers community-based rehabilitation and support services to individuals with severe and persistent mental illness, all under the clinical supervision of the team leader and psychiatrist. 
 Develop comprehensive goal plans in conjunction with individuals and family members. 
 Provide service coordination for an assigned group of individuals, including coordinating and monitoring the activities of the individuals treatment team. 
 Develop, write, implement, evaluate, and revise, as necessary, individual treatment plans in collaboration with the Community Treatment Team. 
 Provide individual supportive therapy, psychotherapy, and symptom management ensuring that immediate changes in the treatment plan are made, as necessary. 
 Act as a liaison and consult with community agencies and families to maintain coordination in the treatment process. 
 Educate and support individuals families and advocate for individuals rights and preferences. 
 Document individuals progress while maintaining a permanent record of individuals activity in accordance with policies and regulations. 
 Provide treatment, rehabilitation, and support services in conjunction with other community treatment team members and with directions from staff with specialty training. 
 Provide on-call crisis intervention as required.  
What is ACT? 
Assertive Community Treatment (ACT) Teams are composed of professionals including psychiatrists, nurses, mental health professionals, substance abuse professionals, employment specialists, housing specialists, and a peer specialist, all of whom collaborate to provide complete services including therapy, medication management, and resource linkage to adult individuals with serious mental illness (SMI). Services are provided in the individuals home or the community and include at least three home visits a week. Will be required to utilize personal vehicles for client transportation and community-based work.

The ideal candidate will possess the following qualifications:  
 Masters Degree in counseling, social work, psychology, or related human service field  
 Active Drivers License  
 Current State and Federal Clearances  
 Knowledge and understanding of recovery principles and recovery driven systems 
 2 years of clinical experience working with adults with severe and persistent mental illness preferred
