JOB REQUIREMENTS: This advanced position is responsible for facilitating
and coordinating a wide range of individualized, client-centered, and
trauma-informed interventions for individuals enrolled in the
Comprehensive Community Services (CCS) and Coordinated Services Team
(CST) programs, in accordance with Department of Health Services
Administrative Rules and Statutes. CCS provides services to individuals
of all ages who need ongoing services for a mental illness, substance
use disorder, or a dual diagnosis beyond occasional outpatient care, but
less than the intensive care provided in an inpatient setting. CST
serves children with emotional disorders who are involved in multiple
systems of care. Responsibilities of this position include performing
intake functions, screening for eligibility, assessing needs, monitoring
ongoing services, and safety planning to reduce the risk of
hospitalization or out of home placement. Responsible for providing
truthful and accurate written and verbal communications. Essential Job
Functions 1. Completes comprehensive assessments and facilitates team
meetings with consumers, providers, family members and other informal
supports to develop, implement, and monitor person-centered,
recovery-focused service plans. 2. Procures and authorizes services to
be delivered by contracted service providers. Monitors the quality and
quantity of purchased services to ensure they are consistent with the
consumers goals set forth on the service plan and in compliance with
authorized hours. Follows up with provider in a timely manner when
adjustments are needed. 3. Prepares and maintains service plans and
progress notes on all assigned consumers; completes all documentation as
required by the Department of Health & Human Services, DHS Chapter 36,
and WI Statute 46.56 which includes daily documentation of all
face-to-face contact, and collateral contacts as well as updating
assessments, service plans and safety plans every 6 months. 4. Completes
Adult Mental Health/Substance Abuse and Childrens Long-term Care
Functional Screen Training to determine program eligibility; conducts
Child and Adolescent Needs Assessment (CANS) to determine needs and
strengths of youth to help guide service planning; completes other state
required program documents and tools as assigned. 5. Assists
youth/families and/or adult consumers in obtaining all available
resources (financial, social, remedial) to enable them to reach
established goals and objectives. 6. Advocates on behalf of consumers to
ensure needs are met and client rights are protected, enhancing natural
supports to the greatest extent possible. 7. Conducts and/or
participates in community awareness, education, resource development,
and option exploration activities. General Job Functions 1. Provides
regular supportive contacts with consumers/families, service providers,
team members and other community partners. 2. Performs other service
facilitation activities as required. 3. Participates in weekly team
meetings to participate in clinical supervision and receive division
program updates. 4. Strictly follow federal and state laws and
regulations, county and departmental policies and procedures in regard
to privacy, security and confidentiality of individuals personal and
health information. OTHER EXPERIENCE AND QUALIFICATIONS: 1.
Two (2) or more years demonstrated experience providing psychosocial
rehabilitative services to individuals with mental health and substance
use disorders in CCS or a related program strongly preferred. 2.
Required to complete training and pass Department of Health Services
exams to become certified as a functional screener and Child &
Adolescent Needs Assessment (CANS) assessor within three months of hire.
