We are currently seeking a Program Coordinator to join our Intellectual and Developmental Disabilities Services (IDD) division at a residential home in Hummelstown, PA.
Position Overview
The Program Coordinator holds primary responsibility for site-based compliance to PA Office Of Developmental Programs regulations, county requirements and implementation of all MERAKEY policies and standards. The Program Coordinator assumes the onsite leadership role in assuring the health, safety and contentment of the individuals served, and the smooth daily operation of their assigned program. Additionally, this position will be the primary supervision on second shift with House Manager and site based Direct Support Professionals (House Manager Aide)
The Program Coordinator holds primary responsibility for site-based compliance to PA Office of Developmental programs regulations, county requirements and implementation of all Merakey policies and standards. The Program Coordinator assumes the onsite leadership role in assuring the health, safety and contentment of the individuals served, and the smooth daily operation of their assigned program. Additionally, this position will be the primary communicator between the House Manager and DSPS . Training, supervision, administrative and management functions may potentially be assigned to include:
Demonstrate competent implementation of all Individual Support Plans (ISPs)
Leads team members in developing community opportunities for the individuals served
Demonstrate understanding of applicable regulations, policies, and procedures.
Assures that all Direct Support Professionals assigned to the site have been effectively trained in BSPS, ISPs, site safety, and applicable regulations.
Maintains a clean and hazard free environment
Accountable for frontline site safety requirements, including safety systems and fire drills and site safety training..
Initial responsibility for accurate daily documentation and oversight of DSP
Is responsible for efficient completion of Personal Property Inventories
Facilitates all medical care, including necessary documentation, with appropriate Medical staff and management staff as needed.
Assists the House Manager with scheduling of Direct Support Professionals for the program as assigned.
Assures effective implementation of individual medication policies and procedures on site.
Monitors Medication Administration Records for accuracy
Assures appropriate transportation for individuals
Completes Monthly Reports as assigned
Manages on site individual funds according to the Merakey policies in conjunction with House Manager
Effectively manages house petty cash and grocery budgets in conjunction with House Manager
Assures that dietary needs of the individuals are met
Assists with completion of annual assessment as requested
Assures the physical site always meets all standards
Efficiently completes Maintenance requests as needed
Completes annual self-assessment specific to site issues
Works additional Direct Support hours as needed when coverage cannot be located
Shares information through the established Chain of Command, providing effective response and follow up as needed.
Assures all documentation is delivered to the supervisor in a timely manner
Presents professionally in dress, demeanor, and communication
Completes all training as assigned
Provides all training as assigned
Attends all supervisory meetings as assigned
Provide Direct Support to the individuals in their charge
Active member of the team in program and plan development
Demonstrate initiative and personal responsibility for the health and welfare of the individual
Respectfully listen to and communicate with consumers, families, coworkers, management, and community members
Protect each persons dignity in the home, workplace, and community.
Demonstrate full knowledge of plans and required supports of each consumer served
Facilitate quality healthcare by being aware of medical issues, transporting individuals as assigned, and effectively communicating with the healthcare professional.
Observe individual behavior/symptoms of mental illness and inform supervisor and/or Psychiatric Doctor of any unpredictable or worrisome behavior.
Provide assessment, observation and documentation related to psychiatric symptoms and the anticipated needs in stabilizing the individuals and their mental health concerns.

Requirements

Minimum HS diploma
Valid PA drivers license
Criminal clearance
One year related experience (some college coursework and supervisory experience preferred)
