Summary The Ambulatory Care Social Work Supervisor has significant program and personnel management responsibilities for the social work services and professional social work staff members serving the Primary Care and Ambulatory Care Specialty clinics at the San Francisco Health Care System. This position provides oversight to all the Northern CBOCs including Santa Rosa, Ukiah, Clearlake, Eureka as well as the Ambulatory Care social work float positions. Responsibilities Supervisory social workers at the GS-13 grade level typically supervise professional and non-professional staff. They are responsible for the professional and administrative management of an area in a social work service, in a care/product line, or across multiple sites, (e.g., multi-division facilities and CBOCs). Incumbent may also provide direct clinical services. Screening/Assessment: The incumbent demonstrates the ability to independently conduct a psychosocial assessment with a wide variety of individuals from various socio-economic, cultural, ethnic, educational, and other diversified backgrounds. The assessment includes documentation of identified behaviors or symptoms of abuse, neglect, exploitation and/or intimate partner violence, as well as strengths, limitations, coping skills, and internal/external supports and service needs to optimize the Veterans functional status. The incumbent develops the assessment in collaboration with the interdisciplinary treatment team, the Veteran, available family members and significant others. The incumbent identifies family/caregiver stressors and provides specific interventions. The complete psychosocial assessment will be completed at admission. Information for the assessment is gathered from the Veteran, CPRS, HOMES, and collateral sources such as family/agencies with appropriate ROIs. Timeframes and requirements for assessments are completed as outlined by Social Work Service policy, and accreditation agencies such as Joint Commission and CARF. All other required screenings are to be completed thoroughly in required timeframes as outlined in program, service, MC or national policy/guidelines. Treatment Planning/Goal Setting: The incumbent possesses knowledge of medical and mental health diagnoses, disabilities and treatment procedures (i.e. acute, chronic and traumatic injuries/illnesses, common medications and side effects, and medical terminology) to formulate a treatment plan. The Individual Service Plan (ISP) includes Veteran strengths, short & long-terms goals with measurable/S.M.A.R.T. objectives, referral plans, and specific history of (mental health, substance abuse, medical, financial, family, sexual, community/culture, legal, and housing). The incumbent coordinates care with internal and external providers as needed. The incumbent reassesses Veteran needs at minimum annually and meets with Veterans as often as needed to provide treatment as outlined in the individual service plan. Referrals/Linkage Services: The incumbent is the subject matter expert regarding VA and/or community resources. After determining Veteran needs, the incumbent is responsible for making appropriate referrals to community and other governmental agencies and coordinating services as needed. Counseling/Therapeutic Interventions: The social worker is skilled in a range of specialized interventions and treatment modalities used in specialty treatment programs or with this specialized population who are experiencing a variety of psychiatric, medical, and social problems to achieve treatment goals, including individual, group and or family counseling or psychotherapy and advanced level psychosocial case management. The social worker is able to incorporate complex multiple causation in differential diagnosis and treatment within approved clinical privileges or scopes of practice. Graduates of schools of social work that are in candidacy status do not meet this requirement until the School of Social Work is fully accredited. Licensure: Persons hired or reassigned to social worker positions in the GS-0185 series in VHA must be licensed or certified by a state to independently practice social work at the masters degree level. Preferred Experience: Two years of previous Supervisory experience; Familiarity with VA and community homeless services; Experience working with Veterans or a related population with complex biopsychosocial needs. Grade Determinations: In addition to the basic requirements for employment, the following criteria must be met for this position. Supervisory Social Worker, GS-13 Experience/Education. One year of experience equivalent to the GS-12 grade level. Experience must demonstrate possession of advanced practice skills in a specialty area or in administration demonstrating progressively more professional competency and judgment. Candidate may have certification or other post-masters degree training from a nationally recognized professional organization or university that includes a defined curriculum/course of study and internship, or equivalent supervised professional experience. Licensure/Certification. Individuals assigned as supervisory social workers must be licensed or certified at the advanced practice level, and must be able to provide supervision for licensure. Demonstrated Knowledge, Skills, and Abilities. In addition to the experience above, the candidate must demonstrate all of the following KSAs: (a) Ability to independently organize work, set priorities, and meet multiple deadlines. (b) Skill in a range of supervisory duties which includes clinical supervision, consultation, negotiation with other departments and quality improvement. (c) Ability to ensure provision of clinical social work services by supervised social workers. (d) Ability to delegate authority, manage priorities, coordinate work, and follow up on pending issues to complete duties in an accurate and timely fashion. (e) Ability to analyze organizational and operational problems, develop and implement solutions that result in efficient operations, and use data effectively to manage workload, quality, performance, and productivity within the service. (f) Ability to provide staff training and development. Assignment. For all assignments above the full performance level, the higher-level duties must consist of significant scope, complexity (difficulty), and variety and be performed by the incumbent at least 25% of the time.
