Spectrum Healthcare Resources has an Exciting Potential Opportunity for a Registered Nurse-Disease Management supporting Wright Patterson Air Force Base in Dayton, OH.
Job Requirement:
Degree/Education: Graduate from an associate degree program in nursing accredited by the Accreditation Commission for Education in Nursing (ACEN), the Commission on Collegiate Nursing Education (CCNE) or national nursing agency that is recognized by the US Department of Education. Bachelors degree in nursing preferred. 4 years full-time specific disease management experience.
Certifications in addition to Basic Life Support: Certification in Disease Management related areas such as: Chronic Care Professional (CCP) by Health Sciences Institute or Certified Health Coach (CHC) by National Society of Health Coaches; or a patient education body such as: Certified Diabetes Instructor (CDE) by National Certification Board of Diabetes Educators or Certified Asthma Educator (AE-C) by National Ashma Educator Certification Board; OR- can be certification eligible with two (2) years of full-time disease management experience.
Experience: A minimum of four (4) years experience in disease management experience is required for consideration of a candidate with an associate degree in nursing. Two years full-time experience in disease management is required for graduates with a Bachelor of Science in nursing. Full time employment in a nursing field within the last 36 months is mandatory.
Board Certification: Certification Board of Diabetes Educators or Certified Asthma Educator (AE-C) by National Ashma Educator Certification Board; OR- can be certification eligible.
Licensure/Registration: Current, full, active, and unrestricted license to practice as a Registered Nurse. Licensed in good standing and cannot be under investigation nor have any adverse action pending from a Nursing State Board or national licensing/certification agency.
Duties: Not limited to;
Utilize major components of population health and disease management, ensure health care interventions for individuals and populations of patients with prevalent and/or chronic diseases to achieve disease management compliance, improved overall quality of life, and reduced healthcare costs. Also performs clinical nursing care in the execution of disease management services.
Coordinate patient care through a continuum and facilitates the achievement of optimal outcomes in relation to clinical care, quality, and cost effectiveness.
Identify and stratifies individual patients and populations of patients across the spectrum of care at risk for chronic diseases and co-morbid conditions and evaluates the incidence of such conditions from a variety of established data bases and facility treatment team referrals.
Facilitate identification, adoption, implementation and utilization of standardized clinical practice guidelines and protocols for management of specific diseases/ conditions.
Identify, develop, and executes appropriate disease management activities and interventions such as, but not limited to, individual and/or group patient contacts, training classes, health promotions, counseling, and education.
Conduct a thorough evaluation of the patients current physical, psychosocial and health status and develops a plan of care based on established CPGs. Documents treatment plans and matches level of patients care to clinical risk readiness to change and health literacy.
