Position Summary:

Provide and supervise comprehensive nursing services and interventions to assure that the health care needs of individuals receiving services are met. Provides continuity of medical care and services to individuals by monitoring the planning, organizing, directing, and controlling nursing activities including those carried out by support staff.

Principle Duties and Responsibilities:

• Assure that the medical needs of the participants are addressed by working with physician(s) as appropriate. Follow through with the provision of nursing care by monitoring nursing and related services.

• Provides for the individual’s continued health, welfare and safety by direction, coordination, and supervising the individual’s medical care plan along with the implementation of all agency and OPWDD policies and procedures that are related to the Rights, Care, Treatment, and Protection of all individuals.

• Ensures that the home is properly staffed with qualified personnel to meet the individuals’ needs by monitoring, training and evaluating all med-certified staff as per Springbrook and OPWDD guidelines.

• Maintain continuity of care by assessing individuals in various settings, receiving reports on the status of individuals, assisting support staff with unusual treatments, deciding when to call physicians, coordinating services with other shifts and taking other actions deemed necessary at the time.

• Provides information to participants, relatives, and employees as appropriate by answering or anticipating questions and requests.

• Promotes harmonious relationships with agencies and persons with whom Springbrook regularly works by careful scheduling and personal contacts.

• Schedule and provide in-service training to staff according to OPWDD regulations on a variety of medical issues and concerns so that staff are knowledgeable and prepared to provide appropriate services.

• Keeps appropriate personnel informed by communicating information, such as complicated medical care situations.

• Serving as model to others in the completion of appropriate paperwork for both billable and non-billable services.

• Maintain a quality program by performing other related duties which may be necessary.

• All other duties as assigned.

Qualifications, Skills and Knowledge Requirements:

• Current certification/licensure by the New York State Education Department as a Registered Nurse

• Must be able to work a flexible schedule.

• Duties require professional verbal and written communication skills.

• Proficiency in or knowledge of using a variety of computer software and e-mail applications, especially Microsoft Excel, Outlook and Word; have the aptitude to learn other computer software as necessary.

• Meet the requirements to drive Springbrook vehicles as required.
