The Pharmacist, Ambulatory Care plays a critical role at CVS Health. Committed to improving the patient experience, the Pharmacist, Ambulatory care helps their patients to understand and feel connected to their health care plan by identifying and discussing medication changes and adherence gaps. As a member of a collaborative care team, the Pharmacist, Ambulatory Care works in partnership with prescribers across a network of physicians and hospitals. Primary Roles and Responsibilities:
The Pharmacist, Ambulatory Care is responsible for but not limited to the following (roles and responsibilities may evolve over time):
Work as part of a clinical care team to improve patient satisfaction, lower medical costs, and improve quality of care metrics
As part of the clinical care team, you will participate in daily huddles with clinicians, medical assistants, and other healthcare professionals and help make recommendations based off reviews of patients’ medications, testing results, and care gaps including accurate EHR documentation
Support patients and clinicians under a collaborative practice agreement to manage certain chronic conditions along with providing other patient care services and coordinating medication reconciliation across a spectrum of care
Support CVS to build out a clinical care model by providing continuous feedback and improvement opportunities for the care model, applying your knowledge to help differentiate CVS from competitors
Develop other Pharmacists and Residents on best practices for operating within a clinical care model
Potentially, as capacity allows, support other CVS Pharmacy initiatives/tasks as needed
Required Qualifications:
Active, unrestricted pharmacist license in the state of clinic location required
1-3 years’ prior relevant experience as a Clinical pharmacist -Ambulatory experience preferred
Bilingual in English and Spanish
Completion of all required training for current role within required timeframe
Ability to stand and walk for an extended amount of time
Preferred Qualifications:
2 years of clinical, ambulatory, or outpatient pharmacy practice experience
Experience working in ambulatory care setting or similar experience
Experience being part of clinical care team
Board certification by the Board of Pharmacy Specialties (BPS) in Pharmacotherapy (BCPS), Ambulatory Care (BCACP), or Geriatric (BCGP)
Experience in primary care: internal medicine, geriatrics, or family medicine
Completion of a Pharmacy Postgraduate Residency (community or managed care PGY-1 and/or ambulatory care PGY-2)
Experience with Medicare / Medicare Advantage plans
Immunization certified or willingness to become certified
Passion for teamwork and the opportunity to collaborate cross-functionally
Education:
Bachelors of Science or PharmD from an accredited college of pharmacy

Pay Range
The typical pay range for this role is:
$60.00 – $73.00
This pay range represents the base hourly rate or base annual full-time salary for all positions in the job grade within which this position falls. The actual base salary offer will depend on a variety of factors including experience, education, geography and other relevant factors. This position is eligible for a CVS Health bonus, commission or short-term incentive program in addition to the base pay range listed above.
