About the Unit/Department/Team The Cardiac Rehab team at Lakewood Health System is a specialized unit dedicated to supporting patients in their recovery and improvement of heart health. Our team works closely with patients, families, and other healthcare providers to offer individualized care that meets each patients unique needs. Mission for Position The Cardiac Rehab RN will provide compassionate, specialized care to patients recovering from cardiac events or procedures. This role focuses on monitoring patient progress, delivering therapeutic exercises, and educating patients on lifestyle changes that promote cardiovascular health. Duties & Responsibilities Design and implement personalized cardiac rehab plans for patients per guidelines Monitor patient progress, vitals, and response to exercise in a clinical setting Provide education on heart-healthy lifestyle choices, including diet and exercise Collaborate with other healthcare professionals to adjust treatment plans as needed Support patients emotionally and psychologically as they adapt to their cardiac rehabilitation programs Position Type Monday – Friday (4 days per week; 8-hour day shift) Qualifications Qualifications Active RN licensure in Minnesota Ideal Candidate Previous experience in cardiac rehabilitation or related field preferred Strong interpersonal skills for patient and family interaction Basic computer skills and familiarity with EMR systems (EPIC experience is a plus) Quality of care focused Willing to take on new challenges Open to learning Enjoys working as a team Benefits Lakewood Health System is growing and looking for great people to join our mission of providing personalized, quality healthcare for a lifetime.
