JOURNEYS COMPANION AIDE

STARTING RATE: $15.00 – $15.00/hr.

JOB DESCRIPTION: Addresses the palliative care, and end of life needs of journeys resident, while supporting the family during the end of life transition which takes place, Maintains a supportive environment for Journeys Residents and their families.

RESPONCIBILITIES AND REQUIREMENTS:

• Accompanies patients/residents journey’s activities as appropriate.• Provide end of life support to families/resident.• Assists in transporting patients/residents.• Provide reminiscence time with appropriate projects/activities, including Journals.• Involves residents with activities both on and off the units and provides one-on-one visitation.• Transports patients/residents to/from dining rooms, therapies and activities as appropriate.• Respects the patient/resident’s dignity, confidentiality and individuality.• Protects the resident’s safety. If unable to remove the danger, notes and reports anything that may cause resident harm or injury to the team leader.• Maintains resident rights.• Maintains HIPPA privacy, security and confidentiality standards.• Follows all established policies and procedures.• Follows universal Precautions and Infection Control techniques.• Maintains Corporate Compliance standards.• Contributes to facility Continuous Quality Improvement initiatives as instructed by the supervisor.• Follows safety procedures by keeping areas free of hazardous conditions.• Economical and careful with supplies and equipment.• Comes to work as scheduled and consistently demonstrates dependability and punctuality.• Comes to work in neat, clean attire and consistently presents an appropriate professional appearance.• This individual continuously exhibits and fosters a spirit of teamwork, positive attitude and conflict resolution among co-workers.• Reports perceived problems to supervisor or as instructed. Contributes ideas to rectify identified problems and follows corrective actions as directed by the supervisor. • Perform duties with measured accuracy and timeliness.• Sets high standards, seeking to “raise the bar”, exhibits commitment to strive for and exceed expectations.• Effectively communicates, assists, and cooperates with other personnel and interdisciplinary teams. Exhibits an attitude of helpfulness, team spirit and support to all team members.• Acts as a source of direction to visitors, staff and external populations, i.e.: delivery staff, vendors, etc.• Represents the organization professionally.• Interacts positively with residents, family members, and visitors under a variety of conditions and circumstances.• Promptly reports all needs and concerns to the Supervisor or designated alternate.• No deficiencies related to Activities and Resident Council as determined by annual State Survey.• Achieve a 90% or higher rating on the resident expectation survey as it pertains to activities.• Reduce the number of residents who have experienced a fall as measured by the facilities Quality Measures.
