Homelessness is the largest social and public health crisis in California. Illumination Foundation (IF) is a growing non-profit organization dedicated towards disrupting the cycle of homelessness by providing targeted, interdisciplinary services in our recuperative care centers, emergency shelters, housing services and childrens and family programs. IF currently has 13 facilities with 22 micro-communities scattered across Orange County, Los Angeles County and the Inland Empire.

Job Description

The Children and Family Program Navigator is responsible for coordinating the care of children and families that are residing in Illumination Foundation’s family emergency shelters. CFNs will support onsite programming, complete screenings, provide referrals and linkages. CFNs will provide follow-up support to families after leaving the program to ensure children participate in ongoing services.

The pay range for this position is $19.00 – $22.00 per hour, depending on experience. 

Responsibilities

Administrative/ Record Keeping

Maintain updated & accurate case notes, records, & relevant paperwork on KIPU & HMIS.

Input and enter all data from screenings on relevant google docs, KIPU, and HMIS.

Utilize the Children’s Registry for entry of Developmental Screenings data and referrals.

Track program services for the purposes of data collection & proof of desired outcomes 

Maintain a daily log and update case files/notes as necessary

Ensure shelter is well managed, clean, and safe for families and guest.

Maintain proper inventory of shelter supplies

Cleaning and preparing rooms for new families entering shelter along with other client spaces such as refrigerator and pantry space

Conduct room checks on a regular basis and on a as needed basis

Collaborate with other departments at Illumination Foundation as necessary and appropriate, including volunteer and communications department.

Complete referral paperwork as deemed appropriate for children and families.

Collaborate with outside partners and agencies when necessary

Direct Services

Complete developmental screenings for all children who enter the shelters (ASQ, ASQ-SE, SDQ,

PEARLS etc.)

Complete intakes for families at family emergency shelters

Assist with leading or providing support in group classes/activities for children and parents

Provide direct supervision and childcare during on-site activities, classes, and while parents are in behavioral health services/case management appointments.

Consult with parents regarding parenting, child development and other familial concerns.

When attending appointments with families advocate for clients relating to housing, health, education, and social and emotional wellbeing.

Provide transportation for client appointments if deemed necessary.

Provide referrals and track linkages to relevant services for children on caseload.

Attend case conferences and supervision as needed.

Collaborate with Children’s Department to provide children’s services to families staying in IF emergency and bridge housing facilities.

Continually update resources for activities and lesson plans

Monitor for signs of emotional or developmental troubles in children and communicate to supervisor

Monitor for signs of emotional, physical, and sexual abuse and communicate to CPS as deemed necessary by law

Prioritize individual and client health and safety.

Be able to meet targets and work under pressure.

Communicate any concerns and feedback to Supervisor with tact and professionalism.

Set and maintain healthy boundaries with clients.

Demonstrate the ability to maintain confidentiality of client information and work related information/materials

Qualifications

Required:

1-2 years relevant experience.

Basic computer skills, including the ability to send and receive emails and summarize data in spreadsheets.

Valid CA Driver’s License and eligibility for company vehicle insurance.

Preferred:

Bachelor’s degree in human services, child development or other related field.
