We are currently seeking a passionate individual for the position of a Full-Time Associate Director of Congregate Care. In this crucial role, you will supervise several Community Residence Supervisors and an Adult Home Administrator.

SPECIFIC RESPONSIBILITIES:

Program Leadership, Management and Supervision

Knowledgeable of all MH contractual and licensure obligations for congregate care programs

Orients, trains and supervises the Administrators of the Congregate Care programs, ensuring accountability to agency and regulatory policies and standards.

Responsible for the planning, administration, ongoing monitoring and accountability of the agencys congregate care programs.

Fosters an environment of trust and cooperation between all program staff and across agency departments

Regulatory & Administrative

Provides fiduciary oversight of all congregate care programming working collaboratively with finance to ensure collection and minimize arrears.

Reviews and updates policies and procedures to ensure compliance with all regulatory authorities as well as efficient and productive operations.

Attends SPA meetings and reviews charts for OMH congregate care sites, ensuring expediated admission process

Monitors residential occupancy status

Responsible for ensuring accurate update of information systems used by congregate care

Ensures that all congregate care facilities maintain compliance with all applicable statues, codes and rules and regulations.

Oversees external audits and monthly reporting of operations of congregate care programs.

Ensures outcomes are compliant with funders expectations

Staffing, Development & Training

Ensures programs operate with sufficient, qualified staff to meet the needs of the residents and the requirements of OMH/DOH and any other regulatory bodies.

Interviews and, where appropriate, makes hiring decisions for positions within the congregate care programs.

Serves as a back-up Administrator in the event of an Administrators absence from one of the congregate programs.

Regularly advances knowledge through participation in internal and external training.

Evaluates staff performance, ensures staff participation in training opportunities and monitors overtime.

Attends appropriate committee meetings, educational seminars and inter-agency meeting

Quality Assurance & Quality Improvement

Participates in internal audits and addresses any deficiencies or areas of concern found.

Ensures incidents are handled and documented appropriately by staff and works with appropriate staff on investigation of any significant incidents, and addresses incident trends

Works collaboratively with maintenance department to ensure that congregate sites are properly maintained (environmental quality) and issues addressed in a timely fashion.

Participates in quality improvement activities, seeking opportunities to enhance the quality of programming within the congregate sites through staff training, collaborations with other services providers and new programming/interventions.

Assumes other duties as required.

QUALIFICATIONS:

Bachelors Degree in Human Services field (Masters preferred) and five years of relevant work experience in the mental health field and a minimum of three of management experience

Experience with congregate care setting preferred

Experience with writing and reviewing policies and procedures required

Valid NYS Drivers License with a safe driving record, access to a personal and reliable vehicle

Proficiency in Microsoft Office Suite Required, Electronic Medical Record Experience preferred

Physical Requirements: remains stationary for long periods of time in the office, moves intermittently throughout the office during work hours, reaching overhead, may push or pull 15-20 pounds daily, repetitive wrist/finger movement, read/write reports and use computer on daily basis, ability to drive to other congregate work sites regularly

POSITION RELATIONSHIPS

Reports to : Director of Residential Services

Supervises : Congregate Care Administrators

Schedule: Monday-Friday 8:30am-4:30pm with on call schedule

Location: Islip Terrace, with frequent travel to congregate care sites in Bayport, Bay Shore, West Babylon and Westbury
