Position Summary

At Stony Brook Medicine, our Social Workers are valuable members of our team, who provide empathic care to our patients, their families and visitors, with the highest degree of integrity and respect. Social Workers apply the principles and practices of their profession to provide integrated social casework, including direct counseling, therapy, advocacy, and other services to individuals, families and groups in a variety of settings. Social Workers plan, implement, coordinate and document treatment for individuals diagnosed with both medical and behavioral health diagnosis, substance use disorder, disabilities; victims and violence and other health care related social issues. Most recently, Social Work has been instrumental in responding to both Social Determinants of Health and Health care equity.

Qualified candidates will demonstrate excellent communication skills, interpersonal skills, knowledge and understanding of patient care and effectively respond to changing patient needs by making decisions based on ethical principles and adhering to our high standard of excellence.

Duties of a Psych Social Worker II/III may include the following but are not limited to:

  • Comprehensive psychosocial assessment of patient and family including determination of level of risk
  • Documentation and referral processes based on clinical need and all regulatory requirements
  • Clinical team case conferencing discussions
  • Patient group facilitator and organizer
  • Person Centered and Collaborative Treatment Planning
  • Discharge planning
  • Outside agency contact and collaboration
  • Communicate risk factors to treatment team
  • Follow up on all mandatory screenings
  • Insurance and hospital contacts regarding admissions/Utilization Reviews
  • Demonstrates ability to be trained in all behavioral health areas including: CPEP, Adult and Child Inpatient units, Consult & Liaison, Mobile Outreach Team and Follow up worker role, for purposes of coverage as needed.
  • Demonstrates understanding of discharge planning and Long Length of stay barriers and systems.
  • Demonstrates teamwork and team leader focus.
  • Demonstrates understanding of data collection and statistics.

Qualifications

Required Social Worker II:

  • MSW from an accredited Social Work Education School.
  • NYS LMSW or LCSW.
  • Minimum of three years post Masters Social Work
  • Minimum 1 year experience in an Inpatient Acute Care Psychiatric setting/Mobile team/ ED
  • Social Work student supervision minimally every three years
  • Understanding and ability to work with patients experiencing substance use disorder
  • Demonstrates excellent written, oral and technical communication skills.
  • Ability to work and coordinate with Psychiatric/Medical Treatment Teams in an integrated health model
  • Ability to identify and work with high-risk behavioral health patients
  • Demonstrates understanding of Psychiatric and OPWDD specific community resources available.
  • Demonstrates strong assessment and psycho-social skills.
  • Working knowledge of Long Island and Metropolitan NY community resources
  • Demonstrates patient first, patient advocacy and service excellence.
  • Oncall recall eligible

Preferred:

  • Experience in substance abuse/counseling intervention, Psych/Medical integration, and Behavioral Health with both adult and pediatric populations.
  • Bi-lingual
  • Minimum 3 years working with child and adolescent population
  • Understanding of requirements and linkage to school-based supports and programs.
  • Understanding of child and adolescent residential application and referral process.
  • Ability to navigate CPS, School District, Foster Care, Guardianship barriers and develop collaborative relationships.
  • Experience utilizing DBT Techniques

  • Meet Regulatory Requirements for pre employment screenings.
  • Provide a copy of any required New York State license(s)/certificate(s).

Anticipated Pay Range:

The salary range (or hiring range) for this position is $84,473 – $107,132 Base

The above salary range represents SBUHs good faith and reasonable estimate of the range of possible compensation at the time of posting. The specific salary offer will be based on the candidates validated years of comparable experience. Any efforts to inflate or misrepresent experience are grounds for disqualification from the application process or termination of employment if hired.

Some positions offer annual supplemental pay such as:

  • Location pay for UUP, CSEA & PEF full-time positions ($3400)
  • Night shift differential ($15,000), evening shift differential ($10,000)
  • Inconvenience pay/supplemental shift differential. ($575)
