JOB SUMMARY:

The LPN Unit Manager ( Resident Care Coordinator) is responsible for the direct and indirect nursing care of all residents on a specific nursing unit.

ESSENTIAL DUTIES AND RESONSIBILITIES:

Demonstrates knowledge of age specific developmental factors specific to the adult and geriatric population in the planning and delivery of care

Demonstrates knowledge, skills and techniques necessary to care for residents with the following needs: therapeutic, neuro and dementia, dialysis, IV therapy, infectious disease, wound care, end of life care, if applicable

· Assures completion of a comprehensive nursing evaluation of a resident’s physical, emotional and mental status upon admission/re-admission and when indicated, as well as, appropriate follow-up as indicated by the evaluation.

· Attends various facility meetings (QAPI, morning report, care plan, wound, etc) as invited or required

Attends all required in-services

· Supervises the staff nurses, nursing rehab assistants, certified nursing assistants, unit clerks (if applicable), and completes annual and probationary evaluations of same

· Supervises the staff nurses in the administration and documentation of medications, treatments and other nursing services, ensure that the medical record reflects progress and the current condition of the resident at all times

· Issues appropriate preliminary disciplinary action to nursing staff as necessary

· Receives report from previous shift, makes rounds and assesses the needs of the residents/patients

· Reports to oncoming shift staff nurse/unit coordinator/RN Supervisor any pertinent information regarding residents

· Ensures that provider (MD, NP, PA) is informed of any change in the resident’s condition, abnormal lab results, abnormal diagnostic test results, consultant recommendations, etc.

· Notifies provider when resident visits are due per regulations/facility policy

· Ensures that provider orders are transcribed accurately and completely, and that medication and lab work are ordered if needed

· Completes quality assurance audits are directed by the DON

· Arranges transportation to and from outside appointments

· Mediates family, visitor and staff concerns

· Notifies the maintenance department of environmental concerns/repairs

· Ensures that the nursing unit is well- stocked with necessary supplies

· Ensures that the comprehensive care plans are revised and updated to reflect a change in condition or provider orders

· Attends family meetings and ensure understanding of the care plan and how it is to be implemented

· Assists the nursing staff in understanding individual resident needs and implementation of their care plan

· Maintains an organized and efficient unit

· Ensures that the responsible party is updated with changes in resident condition

· Participates in discharge planning if indicated (nursing discharge summary, prescriptions, etc)

· Prepare accident/incident reports and detects/corrects situations that have a high risk of causing injury to residents; develops and implements a prevention plan for all incidents occurring on the unit

· Ensures that appropriate information is sent with resident upon transfer, including advanced directives

· Ensures coordination of care across disciplines

· Informs staff of changes in policies and provides education to improve care and safety of residents as indicated

· Assists the Associate Director of Quality and Education and the Clinical Instructor in the orientation of nursing staff and nursing assistant trainees

· Possess knowledge of facility policies and procedures, abides by these policies and ensures that the staff and providers (MD, NP, PA) are informed of and correctly carries out these policies

· Knows location of maintenance free respirators

· Responds appropriately to behavioral, medical and fire safety situations

· Tracks and maintains immunization and infection control logs

· Makes appropriate referrals to the dentist, optometrist, podiatrist, psychiatrist, social worker and specialized therapies to meet the resident’s needs as indicated

· Ability to adjust to changes in unit/shift assignments and meet the resident and facility needs

· Displays optimism fostering a spirit of teamwork, positive attitude and conflict resolution among co-workers. Interacts positively with residents, family members and visitors in all circumstances

· Interacts with insurance companies to ensure that benefits are provided as indicated

· Maintains resident rights

· Maintains HIPAA privacy, security and confidentiality standards

· Follows all established policies and procedures

· Maintains Corporate Compliance standards

· Contributes to facility Quality Assurance Performance Improvement initiatives

· Follows safety procedures by keeping areas free from hazardous conditions

· Is economical and careful with supplies and equipment

· Completes any other duties as assigned by Supervisor
