The Social Worker provides social work service to patients, their families and /or significant others through counseling, emotional support, assisting with environmental needs, crisis intervention, and linkage/referral, and discharge planning.

MINIMUM QUALIFICATIONS:
EDUCATION, CERTIFICATION, AND/OR LICENSURE:
1. Graduate of an accredited school of Social Work or related field.
2. Must be licensed through WVBSWE with minimum of LSW Provision.

PREFERRED QUALIFICATIONS:
EXPERIENCE:
1. Minimum of (2) years experience working in healthcare or in community welfare setting.

7. Collaborates with the case manager for continued care management support as applicable.
8. Attends weekly outlier meetings as scheduled.
9. Delivers and documents Medicare Star ratings given for SNF referrals.
10. Participates in self-evaluation and develops a professional plan for development based on personal goals/strategic plans.
