In Home — 7:00am – 7:30pm, Monday – Sunday (rotating weekends.)
Virtual — 7:00am – 3:00pm & 3:00pm – 11:00pm, Monday – Sunday (rotating weekends.)
Primary Function/General Purpose of Position
Virtual Registered Nurse/Hybrid will alternate work shifts as Clinical RN and the Remote Patient Monitoring RN. This Nurse will also rotate working in direct patient care in the designated care setting and remotely as a Virtual Registered Nurse as designated by their department.
In their Clinical RN role, they will provide nursing care for patients following established standards and practices in accordance with the South Carolina Nursing Practice Act.
In the Remote Patient Monitoring Nurse role, they will provide professional nursing care through skillful assessment, diagnosis, outcomes identification, planning implementation, and evaluation in accordance with policies, procedures, and triage protocols. Clinical expertise and independent judgment based on extensive knowledge, skill, and experience are evident in meeting established standards of care.
Essential Job Functions
Provides direct hands-on patient care following the patient’s plan of care and current standards of practice for Nursing.
Monitor, interpret, and record data from specialized medical technology and equipment.
Uses nursing clinical skills to review labs, patient care visits, and monitor the need for labs and/or appointments, administer medication, and perform treatment interventions in consultation with healthcare specialists.
Coordinate with other disciplines, agencies/companies, families, etc. to ensure that the care needs of the patient are met. Additionally, contact appropriate providers electronically or by other necessary communication channels to update or coordinate care.
Documents all care provided, coordination of services, orders, goals, etc. in the patient’s electronic medical record in an accurate and timely manner.
Actively participates and contributes to the understanding, development, and analysis of health across a broad population and works collaboratively with call center, and clinically integrated network leadership to improve recognized gaps in care or performance in a continuous quality improvement fashion. Collects pertinent data to identify trends and areas of opportunity for improvement.

Hospital at Home Virtual Registered Nurse additional duties:
Monitor, interpret, and record data from specialized medical technology and equipment. Recognizes and appropriately triages patients based on monitoring metrics, patient responses to questions, and additional information provided by patients as appropriate. Directs patients to the appropriate level of care based on the symptoms reported. Demonstrates knowledge and expertise in triaging patients properly such that all patients receive a timely and appropriate response. Collects data pertinent to the healthcare consumer’s health or the situation. Analyze the assessment data to determine the diagnoses or issues.

Licensing/Certification
Currently licensed as a Registered Nurse in the state of South Carolina or holds a current compact/multi-state license as a Registered Nurse in a recognized NCSBN Compact State and is not a permanent resident of SC.
Education
Graduate of an accredited school or college of nursing. ADN or BSN required.
Work Experience
Three years of professional nursing experience in an acute care setting is preferred. Home health, remote patient monitoring, or telephonic care management experience is desired.

For Virtual Registered Nurse/Hybrid working in Hospital at Home, candidate must have a valid South Carolina driver’s license, with a good driving record. This position requires the operation of a vehicle for Roper St. Francis Healthcare business. Due to this job requirement, the selected candidate/employee must possess a valid, current South Carolina drivers license. In addition, the selected candidate/employee must be insurable under the Roper St. Francis Healthcare vehicle insurance plan, which will be verified by reviewing licensure and driving records.
