Description of Duties:The individual in this role will serve as a Community Support Individual (CSI) for the APEX program. The focus of this program is to provide quality case management services and crisis intervention in the School Systems of Crisp and Dooly Counties; as well as in the home and community that supports the youth with a behavioral health disability.
The CSI worker aids the individuals we serve in schools, their own home, and community to allow them to function more safely and independently. Provides individual with a positive role model. Identifies social, economic, recreational, and educational services in the community that will meet individual needs. Maintains as a liaison with other agencies, professionals, government officials and the community when needed. Recognizes potential crisis/emergency situations, analyzes such situations accurately, develops strategies to deal with such situations, and informs the therapist, Supervisor, or other staff when such incidents arise. Medication compliance, dealing with clients aggressive behavior to ensure no harm comes to the client, staff and/or the public. Provides input to primary therapist and other professionals with regard to the development of client treatment plans.
Monitors individuals for compliance with treatment.Assists clients with behavior management training as well as providing skill building to the individual and family.Assists individuals in attending appointments when necessary and may provide transportation. It is the responsibility of the Worker to ensure that all requirements are met to provide safe transportation i.e. proper drivers license and vehicle inspection and adherence to policy 8.2 for vehicle operations. Monitors clients progress and well-being and assists treatment team in evaluating the effectiveness of planned counseling and support.Provides suggestions for the modification of clients plane of care. Provides emotional support and feedback to clients (and their families and friends where concerned). Acts as a positive role model for clients, providing life skills training and behavior management.
Maintains proper documentation of individuals progress or needs In a timely manner. Provides feedback and support to fellow staff and clients. Maintains an up-to-date knowledge of new trends in skill building activities (evidence based) for individuals to use throughout treatment. Provides liaison with other agencies and the community.Fosters positive working relationships with staff members. Provides input to management with regard to the development of the program and participates in program evaluations. Operates office equipment such as:personal computers, photocopiers, facsimile machines, printers and reports or arranges for equipment repairs. Meet minimum billing requirements for caseload. Participate in interdisciplinary team meetings. Participate in supervision and development. Able to efficiently manage a caseload of up to 50 clients. Performs other related duties as required. Transporting individuals served is a requirement.

Minimum Qualifications:
High School Diploma or GED. Valid, Unrestricted Georgia Drivers License. Must be computer literate.
Preferred Qualifications: Bachelors Degree in related field and two (2) years of experience working with youth and/or individuals with Behavioral Health disabilities.
