Job Description:
Job Summary:
The Nurse Care Manager works in conjunction with the internal and external treatment team members to provide education and support to clients who have or are at-risk for complex medical conditions.
Essential Job Functions:
Functions in accordance with Brightli’s Operational Guidelines, and within his/her professional code of ethics.
Accesses emergency services in order to provide rapid aid in case of emotional crisis.
Is sensitive and responsible to management needs, such as data collection, program evaluation, development of grant applications, maintenance of various contracts for services, etc.
Participates in required meetings.
Regularly participates in evaluation of self and assigned staff according to Operational Guidelines.
Is responsive to all Quality Assurance procedures of Brightli, cooperates with utilization review procedures and follows established procedures, rules and regulations of the Clinical Records department.
Performs other special functions that may, from time to time, be outlined and attached hereto.
Develops wellness and prevention initiatives.
Facilitates health education groups by utilizing advanced knowledge of complex medical conditions.
Participate in the initial treatment plan development for all of their Healthcare Home enrollees.
Collaboratively develop a treatment plan of healthcare goals for individuals with co-occurring chronic diseases
Consults with treatment team members regarding patient identified health conditions.
Manage the process of hospital and medical provider admissions and discharges for HH clients
Train patients on complex health conditions regarding medical diagnosis, treatment and medications
Implement, as required, DMH Net Health technology programs and initiatives such as Cyberaccess, Care Manager and IntellectAbility
Monitor and report performance measures and outcomes using sophisticated information systems developed for nurses.
Knowledge, Skills, and Abilities:
Knowledgeable in chronic disease processes in children and adults
Excellent written and verbal communication skills.
Ability to implement new innovative systems
Ability to make decisions based on critical thinking and nursing process
Experience and Education Qualifications:
Active LPN license
Experience with the IDD population preferred
Supervisory Requirements:
None
Employment Requirements:
Successful completion of background check including criminal record, driving record, abuse/neglect and fingerprint check.
Completion of New Hire Orientation at the beginning of employment.
All training requirements including Relias at the beginning of employment and annually thereafter.
Current driver’s license, acceptable driving record and current auto insurance.
