M-F, 8am – 5pm with rotating holidays and weekends
Salaried Nursing position, Full-time benefits.
The Discharge Planner ensures that high quality patient care is provided in a timely and cost effective manner, in the most appropriate setting. Responsible for coordinating the efforts of the inpatient multidisciplinary team; managing the utilization of hospital, patient/family and community resources and facilitating a smooth transition to the next level of care. Practices professional nursing and promotes patient advocacy in accordance with The Nurse Practice Act and the ANA Code of Ethics for Nurses. Maintains a current Ohio Nursing License.

Responsibilities
– Actively collaborates with the attending practitioner, the family and other members of the multidisciplinary team to coordinate an individualized plan of care across the continuum, communicates the plan to key stakeholders and facilitates a safe and timely discharge to the next level of care. (Continuously)
– Utilizes clinical expertise and best practices to ensure high quality care is provided efficiently and cost-effectively. (Frequently)
– Maintains knowledge of regulatory requirements and ensures regulatory compliance for assigned area. Participates in emergency response preparedness as needed. (Occasionally)
– Participates in the training and development of new and existing staff members, including participation in peer evaluation. (Occasionally)
– Engages in workplace community by adopting changes to workflow in accordance with new processes or standards, participating in data collection or auditing, understanding and participating in department Quality Improvement initiatives, and representing the department at relevant meetings and relaying important information back to staff. (Frequently)
– Provides after-hour services as assigned through on-call availability. (Occasionally)
Qualifications
Education: BSN required
Licensure: Current Ohio RN License or compact state license.
Certifications: Pediatric Nurse or Case Management certifications desirable but not required.
Skills: Skills needed in the following areas: communication, collaboration, leadership, multidisciplinary team facilitation, critical thinking and problem solving.
