Rate: $19.20/ hrs. Per-Diem

Location: North Shore/Metro North, MA

Triangle, Inc. was established in 1971 by business leaders and families who believed that we are all people with ability. Through our array of Career Services, we helped people secure or advance 234 careers in Fiscal Year 2016 – many in growth industries like health care and hospitality. Our School-to-Career and Empowering People for Inclusive Communities (EPIC) initiatives equip high school students with disabilities and recent graduates with the tools to start careers and serve and lead their local communities. Our TAPS program provides evidence-based abuse prevention training that empowers people of all abilities to live safe, healthy lives. At our 10 beautiful homes in Malden, Melrose, Peabody, Danvers, Reading, Stoneham, and Saugus, we provide support to many individuals and help them enjoy more independent, dignified lives.

Position Overview:

Triangle, Inc. is currently seeking an experienced and passionate individual to become a member of our team. In this role, you will have the opportunity to showcase your skills, as this role will be heavily focused on providing direct care for individuals with disabilities in a residential home. This is a per-diem positions with no guaranteed hours.

Essential Duties and Responsibilities:

Provide direct care coverage and support for individuals served, responding to individual issues during assigned shifts.

Provide coverage for regular staff during vacancies, sick days, holidays or vacation time.

Support individuals as needed or directed, in areas of personal care, socialization and leisure activities, community membership, personal well-being, household, money management, activities of daily living, asserting their human and civil rights.

Assist individuals in working on areas identified in their Individual Support Plans in conjunction with the efforts of other team members.

Know emergency procedures and basic emergency facts regarding all individual living at the location where coverage is being provided.

If med certified, dispense medications to individuals.

Complete any necessary documentation, including (but not limited to) Communication Log, Case Log, ROADs Charts, and Daily Progress notes, Incident Reports, Medication and Treatment Charts, Medication Occurrence Reports.

Transport and/or accompany individuals to appointments and other community-based activities and events as needed.

Maintain Triangle property in good and sanitary condition.

Additional duties as assigned by the Program Manager.

Accomplishes all tasks as appropriate.

Qualifications:

High School Diploma or General Education Degree (GED)

1 to 3 months related experience and/or training or equivalent combination of education and experience

MA Driver’s License

Medication Administration Certification is preferred but not required

CPR and First Aid is preferred but not required
