Description:
This position will deliver clinical pharmacy services from a remote setting, supporting inpatient care. Key responsibilities include but are not limited to reviewing medication orders, monitoring patient outcomes, review of patient discharge summary, and collaborating with healthcare teams to ensure safe and effective medication use. This role requires ability to work independently, strong critical thinking skills and clinical judgement, effective communication skills and proficiency in electronic medical record systems. The ideal candidate will demonstrate the Mount Carmel mission and commitment to enhancing patient safety and outcomes.
Position Purpose:
The Patient Care Pharmacist, within the scope of the Ohio Pharmacist Practice Act, will provide timely, accurate, cost effective, and patient-centric pharmaceutical care.
What You Will Do:
Coordinate the preparation, distribution, and dispensing of medications as appropriate in compliance with state and federal regulations and laws, accreditation standards, and departmental policies and procedures; directs and assists pharmacy support staff.
Maintain personal compliance and assist in maintenance of departmental compliance with continuous regulatory readiness. Also maintain records related to the transactions of the pharmacy to achieve adequate control and accountability for all drugs, including controlled substances.
Assist in the management of pharmacy inventory by providing information to pharmacy purchasing, removing expired contaminated or deteriorated stock when detected, helping maintain perpetual inventories, and processing non-formulary requests.
Interpret, evaluate and verify all medication orders.
Collaborate with multidisciplinary team in order to assure optimal and cost effective drug therapy. This includes adjusting drug dosing and schedules, recommending the most efficacious therapy, practicing stewardship in antibiotics, performing drug consults and monitoring, and detecting possible adverse drug reactions, food/drug interactions, drug/drug interactions, and allergic cross reactivity.
Minimum Qualifications:
Education: Bachelor or Doctor of Pharmacy required
Licensure / Certification: Current license to practice as a Registered Pharmacist in the State of Ohio.
Experience: PGY1 residency or three years of institutional pharmacy experience in an advanced clinical setting is preferred
Effective Communication Skills
Demonstrates the ability to plan, organize and manage pharmaceutical care, including delegating tasks to and providing direction to other members of the pharmacy team
Performs work independently with minimum supervision, and is effective in a stressful environment
