SUMMARY OF JOB RESPONSIBILITIES:
The Patient Throughput Coordinator’s primary role is to facilitate patient transfers, admission and discharges to restore and maintain patient flow, while meeting performance improvement/quality measures and length of stay benchmarks. Acts as a liaison among Nurses, Physicians, and the Registration/Bed Coordinator to ensure the prompt and accurate placement/assignment of patients to a bed/unit. Collaborates with Nursing and other clinical departments in determining the best placement of a patient with regard to the patient’s clinical needs and the availability of services. The Patient Throughput Coordinator facilitates daily meetings and the resolution of issues that affect patient flow throughout the facility. Collaborates with physicians and other disciplines to achieve length of stay and core measure benchmarks.
QUALIFICATIONS:
Must be able to function independently with excellent attention to details and be able to multi-task and receive information from multiple sources while continually establishing priorities.
EDUCATION:
· Graduate of an accredited school of nursing. BSN required or obtain within 3 years.
· Current RN license in State of Ohio.
· Professional/specialty nursing certification or obtain in one year.
EXPERIENCE:
· At least 3 years acute care nursing experience with stong clinical skills.
· Ability to collate, reduce and interpret data.
· Bed management and utilization skills preferred.
· BLS and ACLS
KNOWLEDGE AND SKILLS:
· Able to multi-task and work across clinical areas and departmnets.
· Developing facilitator and team building skills
· Excellent physician, nurse and interdisciplinary collaboration skills,
· High level problem solving to develop solutions.
· Requires flexibility, effective prioritization and customer service skills
· Computer literacy – Microsoft Office.
PRINCIPAL JOB ACCOUNTABILITIES:
Provides leadership and strategic oversight to ensure the achievement of the Humility of Mary Health Partner’s mission and living of its values.
1. Provides leadership and support to all patient care units through the facilitation and coordination of patient flow and bed utilization.
2. Establishes positive working relationships with Nurse Managers, Assistant Nurse Managers, Team Leaders, ED Care Coordinators and physicians to evaluate current and future requirements for optimal patient flow.
3. Facilitates external transfers from other facilities through prompt and accurate triaging of requests to the most appropriate clinical service.
4. Provides direction to access specialists regarding patient placement to optimize patient flow.
5. Identifies the on-going status of patient care areas related to staffing, census, patient acuity, and expected admissions, discharges, and transfers (ADT) in conjunction with various sources of information including nurse managers, physicians, case management, and nursing staff
6. Utilizes effective & professional assessment skills with Patients, Physicians, Nurses, Case Managers, Social Workers, and Clerical Staff internally and externally for all interactions personally & telephonically to quickly assess patient’s condition and uses collaboration and negotiation skills to place the patient in the appropriate level of care.
7. Applies quick and accurate clinical review of patient’s current and past history to determine patient’s status and proactively inform unit of patient needs (big boy bed, special equipment, language line, etc.).
8. Communicates effectively with all levels of the organization and other publics. Coordinates direct admissions between physician office practices, acute and extended care facilities to assure that these patients receive a bed assignment in a timely manner.
9. Actively participates in multidisciplinary teams/committees supporting patient access and throughput.
10. Complies with EMTALA, federal and state guidelines, and other regulatory agencies requirements.
11. Attends standing meeting related to effective patient flow (bed huddle/surgery huddle).
12. Utilizes recognized guidelines to recommend patient status and level of care to the admitting physicians and policies.
13. Assumes additional responsibilities and other duties as indicated.
Provides planning, direction, assessment and evaluation to ensure patient flow is in adherence with the organization’s mission, values, policies, procedures, goals and objectives.
1. Promotes stewardship of resources while maintaining or improving quality of care.
2. Participates in performance improvement activities that include Emergency department overcrowding and the patient flow process.
3. Exhibits and encourages ownership for the success of the organization by identifying potential barriers to the admission/discharge/ transfer process and coordinates with all appropriate staff to resolve issues in an efficient and timely manner.
4. Collaborates with the appropriate nursing director/management to determine locations for temporary bed locations in a non-emergency department environment when an unexpected surge in patient census is experienced.
Monitors and evaluates quality of care in collaboration with the Director of Nursing/Clinical Services and health center’s performance improvement programs. Takes corrective action when indicated.

3. Monitors resolved problems for reoccurrence.
Establishes productive working relationship with the Medical Staff and ancillary departments.
1. Participates with leaders from management, medical staff, and clinical areas in decision making.
2. Establishes and maintains effective working relationships with Medical Staff and ancillary departments.
3. Uses tact and sensitivity in discussion and planning with Medical Staff.
JOB ACCOUNTABILITIES – Other:
1. Functions as a resource to a staff nurse in direct care.
