We are looking for a Clinical Administrative Coordinator as an addition to the Practice Extend Team. You will work alongside our Interdisciplinary Clinical Team to improve the health outcomes of the patients and providers we serve. The Practice Extend Clinical Administrative Coordinator is responsible for engaging high-risk patients for Comprehensive Medication Reviews with the Clinical Pharmacist, scheduling PCP appointments for patients on their behalf, assisting with administrative tasks, and communicating with provider offices regarding patient medical history and current needs.

This position is full time, Monday – Friday. Employees are required to have flexibility to work any of our 8-hour shift schedules during our normal business hours of 8:00 am – 5:00 pm. It may be necessary, given the business need, to work occasional overtime.

We offer 4 weeks of paid training. The hours of training will be based on your schedule or will be discussed on your first day of employment.

If you are located in OH, IN and KY, you will have the flexibility to work remotely as you take on some tough challenges.

Primary Responsibilities:

Work with a Clinical Care Team to support patient and provider engagement

Engage high-risk patients telephonically to schedule appointments with Clinical Care Team

Utilize multiple technology systems and outreach to provider offices and/or pharmacies as needed to capture pertinent information for patient engagement

Support patients by scheduling PCP appointments on their behalf

Prepare patient cases in workflow management platform for Care Team patient engagement

Facilitate exchange of clinical information between Care Team and provider offices to ensure identified patient concerns and needs are addressed by provider offices

Communicate with provider offices to problem-solve patient issues and/or facilitate communication between practice staff and Care Team

Notify Care Team of relevant information gathered during calls to patient that may affect the patient’s disease state or medication regimen

Develop strong relationships with provider offices participating with Practice Extend program by communicating patient needs

Build rapport and establish trusted relationships with patients via daily outbound and inbound calls

Required Qualifications:

High School Diploma / GED

Must be 18 years of age OR older

2 years of Prior Medical / Healthcare experience in a hospital, pharmacy, or physicians office providing clinical or administrative assistance

2 year experience working with medical terminology

Computer proficiency, to include strong data entry and utilizing Microsoft Office, Outlook, and direct messaging applications (Teams, Webex, etc.)

Must be able to type and navigate a Windows-based environment simultaneously

Ability to work Monday – Friday, 08:00AM – 05:00PM, with flexibility to work until 07:00PM (two days a week).

Ability to Travel up to 10% of the time

Preferred Qualifications:

Pharmacy Tech, Pharmacy Tech for Insurance Company, or Medical Assistant

Managed Care Experience

Bilingual fluency in English and Spanish

Telecommuting Requirements:

Reside within OH, IN and KY

Ability to keep all company sensitive documents secure (if applicable)

Required to have a dedicated work area established that is separated from other living areas and provides information privacy.

Must live in a location that can receive a UnitedHealth Group approved high-speed internet connection or leverage an existing high-speed internet service.
