Job Summary:
The Charge Nurse is a registered professional nurse responsible for ensuring the quality of patient care in a home
health setting. This role involves coordinating care with the interdisciplinary team, patients, families, and referring
agencies. The Charge Nurse administers skilled nursing care to patients of all ages in their residences and
ensures the continuity, appropriateness, and quality of services provided. The position also includes directing
clinical staff in accordance with agency policies and procedures, and it may be referred to as Nursing Supervisor.
Job Qualifications:
o Education: Graduate from an accredited diploma, associate, or baccalaureate nursing program.
o Licensure: Active, unencumbered state license as a registered nurse, or a multi-state license from a Nurse
Licensure Compact (NLC) member state. Current drivers license required.
o Experience: Minimum of two years as a registered nurse in a clinical care setting and one year in home health
care. Management experience is preferred.
o Skills: Proficient nursing skills as defined by generally accepted standards of practice, strong interpersonal skills,
and proof of current CPR certification.
o Transportation: Reliable transportation with valid auto liability insurance.
o Environmental and Working Conditions: Work in patients homes under various conditions; potential exposure
to blood, bodily fluids, and infectious diseases. Must be flexible with scheduling and local travel; may face
unpleasant weather and PRN emergency calls.
o Physical and Mental Effort: Requires prolonged standing and walking, ability to lift up to 50 pounds, and move
patients. Must handle stressful conditions, make quick decisions, and meet individualized psychosocial needs.
Good hand-eye coordination and manual dexterity are essential.
Essential Functions:
o Supervise and provide direction to clinical nursing and aide personnel, ensuring quality and continuity of care.
o Conduct staff competency assessments, including evaluations of aides.
o Promote the agencys philosophy, mission, and administrative policies.
o Commit to ongoing professional growth and competency.
o Continuously assess home health patients to identify their physical, psychosocial, and environmental needs,
documented through clinical records and team reports.
o Collaborate with patients, representatives, and caregivers to develop and evaluate individualized plans of care.
o Perform patient care and staff management tasks competently.
o Coordinate services and communicate changes in patient conditions to physicians and relevant personnel.
o Integrate and ensure the execution of physician orders into the plan of care.
o Administer services as prescribed in the plan of care while maintaining infection control protocols.
o Provide counseling and education to patients, caregivers, and families.
o Maintain accurate clinical notes to ensure quality patient care and documentation.
o Promote compliance with fiscal intermediaries and other third-party payors through education and support.
o Monitor systems for identifying issues and report them to the Supervising Nurse.
