Job Summary

Community Health Workers (CHWs) are frontline public health workers who have a close understanding of the community they serve. This trusting relationship enables them to serve as a liaison/link/intermediary between health/social services and the community to facilitate access to services and improve the quality and cultural competence of service delivery. Under indirect supervision, this individual would work closely with medical providers, primary care teams, and social services agencies to provide care coordination and connection to resources and support to program clients to improve their health and general well-being through education and provision of coordination of care and services. This individual would work in both clinical and community-based settings.

Education and/or Experience

High school diploma or GED required.

Bilingual in English and Spanish languages required.

A minimum of two years’ direct experience preferred.

Experience with navigating local medical and social support systems preferred.

Possible bilingual and medical interpreter qualifications preferred based on specific position needs.

Possible knowledge and/or experience with certain medical diagnoses preferred based on specific position needs.

Must be from the community that is being served or have a familiarity of the community.

Licensure/Certification/Registration

Medical Interpreter Certification and Medical Terminology course required within 1 year of hire or when available.

Must have valid driver’s license and reliable transportation.

Must compete the Community Health Care Worker certification program within 2 years of hire.

Work Schedule

Monday – Friday, 8:00am-4:30pm; 80 hours every 2 weeks

Pay

The hourly range for this position is listed below. Actual hourly rate dependent upon experience.

$19.25 – $24.25 per hour
