Job Summary: As a Licensed Practical Nurse, you will provide health care services to individual program participants in efforts to achieve overall personal wellness. You will assist in the development of health care plans for all individuals, record medical appointments, dietary changes, conversations with doctors, seizure activity, accident or incident reports, immunizations, treatment programs and all other related health care items in each individuals files. You will monitor services and interpret information found in health care logs to ensure quality of care is provided and that regulatory requirements are met. You will work closely with teams to ensure consistency in services. You will train Direct Service Professionals (DSP) in health care plan procedures as required by regulations. Incumbent will provide primary health care services for up to 25 individuals and may provide consultation on up to 50 individuals. Job Requirements: Licensed to practice as a Licensed Practical Nurse (LPN) in the state of Minnesota. A minimum of one year experience working in a health care facility and/or community nursing experience. Experience working with vulnerable populations is desired. A valid drivers license, acceptable driving record and a Motor Vehicle Check Successfully meet the requirements of a background check. Training in physical assessments and documentation, intellectual disabilities, psychotropic medication monitoring, behavior management, lifts and transfers, First Aid, CPR, and Blood Borne Pathogens.
