Job Title: HIV Prevention Coordinator

Summary:

The HIV Prevention Coordinator reports to the Ryan White Program Manager at Penn Community Practice/Penn Presbyterian Medical Center. The Coordinator will work closely with leadership in the Division of Infectious Diseases to create a Post Exposure Prophylaxis (PEP) Center of Excellence (COE) at Penn Medicine. They will ensure that individuals across the city who experience non-occupational exposures to HIV are linked to PEP services within 72 hours and receive appropriate follow-up medical care. The HIV Prevention Coordinator will work closely with the Emergency Department and other key points of entry (Urgent Care, primary care) that see patients for acute, non-occupational exposure and assist in troubleshooting issues that could arise around access to medication treatment and follow up medical care for PEP and PrEP. They will also screen all patients and assess for the need to transition to Pre-Exposure Prophylaxis (PrEP) and link to care. The Coordinator, along with the Ryan White Program Manager, will provide daily oversight to HIV Prevention Navigators who work with patients on PEP and PrEP. The Coordinator will participate in Ending the HIV Epidemic efforts at Penn Medicine and the City of Philadelphia. The Coordinator is responsible for implementing and assuring that proper documentation and activities are in compliance with appropriate agency, city and funding regulations as stipulated by the Philadelphia Department of Public Health.

Responsibilities:

Participate in the Ending the HIV Epidemic efforts at Penn Medicine and the City of Philadelphia.

Work with Emergency Departments to create a model to ensure appropriate patients are offered clinically appropriate PEP and PrEP.

Work with internal and external pharmacy partners to set up plan to provide 24/7 access to PEP

Provide patients with a comprehensive explanation of HIV/AIDS, STI’s, PEP, and PrEP.

Provide daily oversight and guidance to HIV Navigators fielding calls to PEP COE.

Develop standard operating procedures for the PEP COE

Execute high-level decision making and demonstrate critical thinking skills.

Handle issues independently

Conduct an accurate patient-centered, cultural, sex, and gender-sensitive risk assessment with clients and thoroughly explain the importance of engagement in HIV prevention services.

Participate in community-based programming, including (but not limited to) health fairs, AIDS Education Month, and other organizational/departmental priorities.

Provide emotional support, counseling, crisis intervention, and advocacy for clients throughout the process of testing for HIV infection as needed and make referrals to PrEP providers for those who are HIV negative, including partners of patients living with HIV.

Provide comprehensive information on the risks and benefits of HIV testing and provide a through explanation of the testing process.

Assist in the tracking and distribution of condoms and safer-sex materials

Provide clients with all aspects of pre- and post-test counseling, including but not limited to: actual test results, direct referrals, counseling and emotional support.

Answer phone calls from the PEP COE and respond to emails from the general public and Penn Medicine community by providing information around HIV prevention, treatment, PrEP, PEP, STI prevention, STI treatment resources, and other relevant sexual health topics

Facilitate and ensure that patients the navigator links to care receive the standard of care for medical care associated with HIV.

Submit monthly statistical/narrative reports as required to the Ryan White Program Manager for review.

Regularly attend and actively participate in internal staff meetings, staff training sessions, and external meetings from AACO and community partners.

Advocate for clients as needed.

Refer to Penn Community Practice, MacGregor ID Practice, or medical site of their choice for all clients with reactive HIV test results and those at high risk for HIV infection for PEP and PrEP, for proper follow-up of needed services.

Comply with HIPAA regulations and universal precautions for healthcare workers.

Reports all incidents to the Ryan White Program Manager

Remain updated on all aspects of HIV Prevention and HIV including approved medical treatments, new prevention methods, etc.

Performs duties in accordance with Penn Medicine and entity values, policies, and procedures

Other duties as assigned to support the unit, department, entity, and health system organization

Take after-hours calls to PEP hotline as required

Create and manage call schedule for COE, including after-hours call schedule of exempt employees to ensure 24/7 call center coverage

Create and manage back-up schedule for COE, including back up for after-hours call schedule of exempt employees to ensure 24/7 call center coverage

Education or Equivalent Experience:

Bachelor of Arts or Science (Required)

4 years experience in Social Work, Public Health or related field (Required)
