Turning Points

Position Summary: A Care Navigator would serve as a support person, advocate, and guide to those individuals and families with critical needs outside existing services offered at Turning Points. The Care Navigator will not only connect clients to appropriate resources within the community, but they will also serve as an advocate and call on the clients behalf. They will make appointments, make warm referrals, stay in contact with clients to ensure they are following through with the plan and not encountering further barriers. The goal of the Care Navigator is to improve access to services, enhance coordination, and assist individuals in overcoming barriers to receiving timely, high-quality services.

Essential Duties and Responsibilities:

Performs the following functions, that include but not limited to:

o Provides support, advocacy, and navigation services to individuals and families

o Identifies and assesses client and family needs.

o Guides and informs clients of available community resources.

o Assists new clients in completing intake forms and inputting date into CSIS and other data collection sites.

o Performs intake assessment with clients with care and confidentiality.

o Advocates on the individuals behalf and coachs clients to effectively advocate for themselves.

o Provide direct referrals and assure linkages for services based upon eligibility.

o Support and maintain collaborative relationships with key community service. providers and stay abreast of information pertinent to a successful referral and linkage.

o Enter data into the Community Services Information Systems (CSIS) in a timely manner.

o Must be knowledgeable of community resources systems.

o May monitor volunteers and/or interns.

o On campus outreach.

Qualification Requirements:

o High School or Equivalent education required.

o Must have at least 2 years working within human services field.

o Spanish speaker preferred.

Skills Required:

o Effective written and verbal communication skills

o Good computer skills including Word, Excel and Google Suite.

o Planning, project management, organization and time management skills

o Must be dependable, self-motivated, and able to work with minimal supervision while balancing working as a team.

o Solve practical problems and document results using sound judgment.

o Ability to work with multiple agencies in coordinating, planning, and delivering advocacy.

o Be energetic, passionate and adaptable with a deep commitment to social service, empathy for individuals and families and a positive approach to embracing and managing change.

o Interact effectively with individuals from diverse backgrounds.

o Process and provide feedback in a manner that is sensitive to cultural, linguistic, and psychological needs of participants.

Supervision:

This position reports to the Community Resource. The Care Navigator is expected to use and is given considerable latitude for discretion, independent judgment and self-directed time management in planning and executing work to achieve the most effective results. Supervision is minimal concerning day-to-day activities. Work is reviewed retrospectively, primarily in terms of results achieved and the maintenance of effective and harmonious work relationships both internal and external.

The physical demands described here are representative of those that must be met by an employee to perform the essential functions of this position successfully.

This is an office work environment requiring limited physical effort. Position requires visual acuity for reviewing, checking, preparing and maintaining written and computer files. Employee is required to have sufficient hearing to accurately perceive information at normal and spoken word levels. Must be able to demonstrate proficiency with Microsoft Word, Excel and other required computer programs and databases.

Mental Demands:

Must handle new and diverse work problems on a daily basis. Personal maturity is an important attribute. Must be able to resolve problems, handle conflict, and make effective decisions under pressure.
