OCCK, Inc. is looking to add a Transition Specialist to their team.
This specialist works directly with customers to meet their needs and
get back into the community. This will include in home visits, shopping,
contacting community resources, phone calls, data entry, document
review, and other duties such as:
– Providing assistance in identifying and securing a safehome in the
community
– Providing support in obtaining furnishings, household items, DME and
items necessary for transitioning to live independently
– Assistance with finding community resources and supports for HCBS
services
– Create a budget with customer of deposits, items needed, monthly
rent for a successful move back into the community
– Providing follow up after move is complete: three days after move,
14 days, and 30 days after move
– Coordinate move date: landlord, utilities, HCBS services, deposits,
household items, medications, follow up doctors appointment
– Provide training and materials for customers to understand their
rights as they leave the nursing home and back into the community
Position Requirements
– Must have strong oral and written skills
– Strong customer service skills; able to work with a diverse group of
people
– Work independently and also as part of a team
– Posses a strong belief that all people have the same rights and can
live independently
– Ability to teach others about their rights and responsibilities
– Willingness to explore resources that are needed for consumer to be
independent
– Must meet the eligibility requirements of the auto insurance company
providing coverage for OCCK vehicles
– Flexible schedule to meet customer needs, home visits and travel
required
– Valid Kansas Drivers License or have access to transportation
Full-Time/Part-Time: Full-Time
Pay Rate: $18.00 per hour
