OCOs Crisis & Development Services assist people in gaining safety, self sufficiency and personal growth through prevention, intervention and skill building services. The Health Education Advocates will focus on adolescent pregnancy prevention services including the delivery of comprehensive sexual health education in various community settings including middle and high schools. Health Education promotes inclusivity and diversity with a strong focus on serving the LGBTQ community with culturally competent services and safe spaces.
JOB SUMMARY:
Provides direct services to people of all ages, according to OCO Universal Service Delivery methods in diverse locations, including OCO offices, other agency offices, program sites, public places and the homes of consumers. Independently provides early intervention and care management services to an assigned population and oversees project functions as assigned. Carries out duties in accordance with Agency and Program policies and procedures.
JOB DUTIES AND RESPONSIBILITIES:
Works with individuals seeking reproductive health service, experiencing sexual health issues, and/or seeking other health services.
Is responsible for the case files of up to 50 individuals & families.
Responds to consumers who are contacting in person, through a variety of multimedia avenues and on the phone to provide information about services & the status of their services.
Works as part of a team to provideone on one education, assessments, and education to increase health outcomes, provide services, support and follow-up.
Independently assesses the additional health & education needs of individuals and families.
Advocates, accompanies and/or negotiates on behalf of consumers.
Provide trainings, monitoring and ongoing support to volunteers, staff, and community partners
Manages services for consumers and facilitates case management meetings with other providers.
Works cooperatively with schools, community members, agencies and other service providers.
Assists manager in organizing and facilitating partnership meetings to advance the objectives of the project
Tracking and compiling statistics related to program activities and outcomes
Completing reports for various project stakeholders
Providing and arranging adequate, up to date training for community partners
Staying up to date with best practices in the field and sharing information with project partners and co-workers
Develops and maintains effective working relationships with community partners, such as schools, medical providers, and other services
Assists in training community partners on STD/HIV prevention, LGBTQ Inclusivity, Teen Pregnancy Prevention, Trauma Informed Care, Positive Youth Development & other adolescent health topics.
Works cooperatively with volunteer staff and utilizes a core of volunteers and interns appropriately to assist in providing survivor friendly services and atmosphere.
Carries out other special projects as assigned.
JOB REQUIREMENTS:
Have a desire to work with individuals and families and has a thorough understanding of the family development process and the OCO Service Delivery system.
Must be able to work independently and be self-motivated, resourceful, organized and adaptable in carrying out job duties
Must be familiar with the geographical layout of the county and have knowledge of human service resources in the county
Must have good oral and written communication skills and have excellent documentation and reporting skills.
Must possess skills for complex data entry and have the ability to read and interpret complex information and manipulate data.
Must have demonstrated ability to work with others in a warm, non-judgmental manner and be a positive role model.
Must be able to direct the work of others, set priorities, and problem solve.
Must exhibit professionalism, good judgment and maintain strict confidentiality with regard to client information.
Must exhibit a detailed knowledge of regulations, services, rights and responsibilities for the population served.
Must have the ability to work with populations with special needs, including those that may exhibit aggressive behaviors.
Must be flexible with work schedule in order to meet the needs of the program.
Must have acceptable health to carry out the responsibilities of the position.
Must have a valid NYS Driver’s license with driving record within policy and access to a properly insured, reliable vehicle for regular travel and transporting consumers.
MINIMUM QUALIFICATIONS:
Must have a Bachelors Degree in a Human Services area; and
Must have 1 to 3 years experience with children, youth, families or a special population; or
Any equivalent combination of education, experience and training.
Full Time – 37.5 hours per week with flexibility required
Grade 9
Monday through Friday – 8:30 am to 4:30 pm
