The Manager, Care Coaching assesses and evaluates members needs and requirements to achieve and/or maintain optimal wellness state by guiding members/families toward and facilitate interaction with resources appropriate for the care and wellbeing of members. The Manager, Care Coaching works within specific guidelines and procedures; applies advanced technical knowledge to solve moderately complex problems; receives assignments in the form of objectives and determines approach, resources, schedules and goals.
The Manager, Care Coaching employs a variety of strategies, approaches and techniques to manage a members health issues. Identifies and resolves barriers that hinder effective care. Ensures patient is progressing towards desired outcomes by continuously monitoring patient care through use of assessment, data, conversations with member, and active care planning. Decisions are typically related to resources, approach, and tactical operations for projects and initiatives involving own departmental area. Requires cross departmental collaboration, and conducts briefings and area meetings; maintains frequent contact with other managers across the department.

Required Qualifications
Bachelors Degree in a health related field
2 or more years of managed care experience in Medicare and/or Medicaid.
1 or more years of management/leadership experience.
Proficiency in analyzing and interpreting trends
Comprehensive knowledge of Microsoft Word, Excel and Access
Progressive experience in the health solutions industry, with emphasis with leading and managing teams
This role is part of Humanas Driver safety program and therefore requires an individual to have a valid state drivers license and proof of personal vehicle liability insurance with at least 100,000/300,000/100,000 limits.
Preferred Qualifications
Masters Degree in human services, social sciences, or health related field.
Applicable State licensure in field of study
RN or LPN with current State Licensure
Proficiency in analyzing and interpreting financial trends
Previous experience in utilization management, discharge planning and/or home health or rehabilitation
Experience with health promotion, coaching and wellness
Health Plan experience
Previous Medicare/Medicaid Experience

Pay Range
The compensation range below reflects a good faith estimate of starting base pay for full time (40 hours per week) employment at the time of posting. The pay range may be higher or lower based on geographic location and individual pay will vary based on demonstrated job related skills, knowledge, experience, education, certifications, etc.$69,800 – $96,200 per yearThis job is eligible for a bonus incentive plan. This incentive opportunity is based upon company and/or individual performance.
