Consultation services provided by a Qualified Professional (QP) for Community First Services and Supports (CFSS) in Minnesota involve offering guidance, oversight, and support to individuals receiving CFSS. The QP helps participants understand their service options, develop a person-centered plan, and manage their services. Essential Job Functions: Adhere to all Arc Northland Policies and Procedures as outlined in Arc Northland s Employee Handbook. Must pass Department of Human Services Background Study Assist Consultation Services participant or their participant representative to create their service plan based on the needs of the participants service agreement. o Assessment and Planning: Assisting participants in identifying their needs and developing a personalized care plan. o Guidance: Providing expert advice on selecting and managing direct support workers and other service-related decisions. o Monitoring Services: Ensuring that the participants services are delivered in accordance with their plan and in compliance with CFSS guidelines. o Support and Training: Offering training and resources to participants on how to manage their services effectively and use CFSS to meet their goals.
