Summary:

The primary purpose of this position is to increase enrollment of potential participants to Catholic Healths LIFE Program, a Program for All-Inclusive Care for the Elderly program while incorporating the philosophy and mission of Catholic Health. This position will be responsible for receiving pre-enrollment information from individuals who express an interest in the LIFE program. This includes providing an explanation of LIFE, eligibility requirements, the philosophy and mission of the program as well as the services provided. Conducts facility tours, in-home intake assessments and program education. Works collaboratively with Community Liaison to build relationships with community referral sources and other key stakeholders. Must possess strong interpersonal communication and customer service skills related to a geriatric population.

Responsibilities:

EDUCATION

Associates Degree or a minimum of five (5) years of experience in relevant field required

Bachelors Degree preferred

EXPERIENCE

Two (2) years of healthcare/social services/human services preferred

Two (2) years of customer service or sales experience required

One (1) year of experience working with frail elderly population preferred

KNOWLEDGE, SKILL AND ABILITY

Working relationship/knowledge of senior health issues and the Programs service area

Fluency in Spanish preferred

Must possess a valid drivers license and have a car available for work use

Strong computer skills required including word processing, spreadsheets, presentations, and data base management

Willingness to learn new software applications

Strong written, verbal and interpersonal communication skills

Ability to work independently.

Ability work productively with all levels of personnel including physicians, families and care givers
