Position: Care Managers-Metro West and South of Boston As key members of
a Care Team, the Care Manager works with clients and providers
face-to-face, over the phone, and through telehealth to identify and
address social determinants of health, build up and engage the social
support network to help our clients and families navigate through
healthcare and other services to support them where they live and
through transitions. Northeast Care Managers believe that improving
social determinants of health, connections, and an understanding of
their disease processes and their goals will improve health outcomes for
their clients. Care managers will work hard to develop strong
relationships with those involved to leverage available resources to
improve their health and well-being. Position Requirements: The 2
positions cover Metro West and South of Boston with frequent daily
automobile travel to meet with clients where in their homes, medical
facilities, and/or their communities. Self-starter with the ability
to work independently with minimal supervision. Ability to show
empathy and quickly build relationships with patients and local
community-based organizations. 2 years of previous experience
working in care management, home health, or other healthcare
environments. Ability to take calls remotely on some nights and
weekends based on your clients needs. Excellent verbal
communication skills both in person and on the phone. Familiarity
with Microsoft Office, Google Suite, and other web-based applications.
Roles and Responsibilities: Perform in-home and telehealth care
management visits to assess and impact social and behavioral status.
Work closely with the client and family to ensure continual progress on
all care management goals. Assess social determinants of health needs
and develop care plans and recommendations. Perform behavioral,
environmental, and social support assessments as needed. Engage
family and social support groups in the education and care of patients.
Assess clients and refer them to behavioral health specialists.
Help patients understand, accept, and follow medical and lifestyle
recommendations. Serve as the point of contact for patient questions
regarding social and behavioral items. Facilitate conversations
around and consideration of proactive care decisions. Review and
document patient updates and progress in the care management platform
timely. Identify and build relationships with referral sources.
Introduce patients to appropriate resources and act as the patient
advocate. Support Northeast Care Management as necessary with any
tasks required to deliver excellent personalized care and perform all
other duties as assigned. Uphold the mission and values of Northeast
Care Management in all aspects of your role and activities.
Qualifications: Must have a valid drivers license, available car,
and be willing to travel in the area assigned. Must be able to pass a
CORI check. Must have computer skills and be familiar with Google
Suite and Microsoft Office. Prolonged periods sitting at a desk and
working on a computer. Must be able to lift to 15 pounds. Travel
Required daily throughout the service area. Travel to training and
meetings as needed.
