CARE COORDINATOR

– Coordinate and facilitate Child and Family Team meetings.
– Lead the development, implementation, and evaluation of individualized services plans.
– Develop trust and mutual respect with youth/families and their team.
– Assist team to identify needs and create a shared vision.
– Assist teams in identifying, recruiting, and engaging natural supports.
– Enable youth/family and team to take ownership of the Wraparound process.
– Support youth/family in identifying their strengths and building upon them.
– Identify barriers to full participation in the program service process and develop strategies to overcome them.
– Prepare summary documents and final reports.
– Ensure that all required confidentiality and other compliance documents are current for each youth.
– Educate families, providers, and community about the elements of the program and its processes.
– Contribute to and support a positive, team-oriented work environment.
– Complete documentation of services and treatment plans consistent with clinical and administrative policies and procedures.
– Work in collaboration with direct care staff supervisors, clinicians, and the family to build rapport and overcome challenges.
– Attend MCO meetings on a weekly basis to discuss and provide updates on clients.
– Assist with referrals/linkages.
– Facilitate monthly treatment meetings for assigned caseload.
– Conduct thorough assessments of clients situations including mental health status, safety risks, and immediate needs.
– Develop and support implementation crisis intervention plans, providing appropriate resources and referrals to support clients in crisis.
– Ongoing case management with a focus on screening and making appropriate referrals to address childs needs in the following domains: Placement issues, safety issues, education, health, mental health, legal/immigration proceedings, substance abuse, independent living, criminal/juvenile justice, child enrichment activities.

Educational Requirements

Bachelors Degree from an accredited university, in a Human Services related field; masters degree preferred. At least two years of experience providing services for children and families.

Physical Requirements

This position will require a wide variety of physical tasks such as standing, walking, running, stooping, bending climbing stairs, and lifting individuals, which requires strength, coordination, endurance, and agility. In addition, there may be occasional contact with hostile clients, and incumbents may be subjected to physical and verbal abuse while executing physical or protective interventions. The employee must maintain sufficient physical ability to ensure safety, in the event a client becomes violent, and attempts to hurt themselves or others. The employee must be agile enough to control and rapidly extricate themselves and clients from buildings in case of emergency.

Additional Requirements

Must be 21 years and older and have a valid NC drivers license with a good driving record. Must be able to demonstrate effective self-regulation strategies. Must be coachable and willing to learn. All potential job candidates must pass a drug screening and background check.
