following opportunity. Description Lutheran Social Services of WI and
Upper MI is currently seeking a full-time, benefit eligible, Forensic
Care Coordinator to join our growing OARS (Offering Avenues for Re-Entry
Success) serving St. Crox, Dunn, Barron, Chippewa and Polk Counties. Our
team provides case management for those that have been convicted of
crimes, are in prison and have verifiable mental health diagnosis. This
role collaborates closely with the Department of Corrections to help
individuals released from the correctional institution reenter the
community successfully. Colleagues are required to meet clients in the
prison system and in the community to perform assessments,
investigations/screenings to determine entry and placement in the
Forensic Opening Avenues for Reentry Success Program. Hours for the role
are generally Monday through Friday and require occasional evening

Duties and responsibilities: This list of duties and responsibilities is

responsibilities, as management may deem necessary from time to time.
Assess clients needs utilizing the required assessment tool(s) (e.g.
functional screen). Re-assess, as needed, and at least annually.
Information to complete the assessment is gained via interviews,
collateral contacts and prior records. Working alongside the client,
their family, his/her supports, and other service providers, develop and
implement a comprehensive and individualized service plan. When required
by the program/service, coordinate and authorize services according to
the service plan. Provide services to assigned clients in alignment with
