Overview:Adoration Health is seeking two dedicated Licensed Practical Nurses (LPN) for full-time positions in Covington, TN. This role offers a consistent 36-hour workweek, where you will provide essential care to a patient as part of a specialized care team. Schedule:

Saturday, Sunday, Monday (7:00 AM – 7:00 PM)

Full-time: 36 hours per week

Responsibilities:

Provide high-quality nursing care, including medication administration and health monitoring, in a one-on-one home care setting.

Maintain accurate documentation of patient conditions and care provided.

Collaborate with the healthcare team and patient’s family to ensure comprehensive, personalized care.

Responsibilities

Perform selected nursing procedures within the scope of LPN practice

Provide patient and family teaching according to plan outlined by the RN

Monitor patients vital signs and make specific measurements and observations

Monitor and record intake and output. Report significant changes immediately

Observe, assess and record patients physical and mental condition and reports any changes

Administer medications as instructed and as ordered by the physician and instructed by the RN and document the patients file accordingly

Attend and participate in multi-disciplinary patient case conferences, staff meetings and in-service education programs as assigned

Qualifications

Licensed Practical Nurse / LPN license in good standing

Previous private duty, home health, hospice, hospital or skilled nursing experience preferred

Active CPR certification

Familiarity with electronic health records

Strong problem solving and critical thinking skills

Reliable transportation, valid drivers license and current automobile insurance
