The Anti-coagulation RN Care Coordinator works collaboratively with the referring providers, ACC Pharm-D, front and clinical staff, IHI ACC Clinical Manager and IHI Practice Manager in delivering efficient, compassionate, and high quality care. The Anticoagulation Clinical Nurse Specialist will assess patients whom are on anticoagulation medication and make dosage adjustments per protocol based on Point of Care Testing (POCT), or laboratory values. They will document in Epic, provide detailed ongoing education and in services to patients, family members, physician, nurses other health care professionals regarding drug therapy and anticoagulation management in order to assure high quality, cost effective and anti-coagulation drug therapy.

Required Qualifications:
Graduate of accredited RN nursing program.
Upon hire: Montana Registered Nurse License
Upon hire: National Provider BLS – American Heart Association
2 years Clinical RN experience, resulting in excellent clinical skills and judgment.
Preferred Qualifications:
Bachelors Degree in Nursing.
Upon hire: Certification in CACP, CVRN, or Med-Surg
Recent Anticoagulation Management, Cardiology or Med/Surg experience.
Decision support experience, Electronic Medical record (EMR) experience.
