Job Posting Title:Private Duty LPN – Full-Time – Hornbeak, TN (50 Hours/Week)

Overview:Adoration Health is seeking a skilled Licensed Practical Nurse (LPN) for a full-time position in Hornbeak, TN. This role involves providing nighttime care for a patient requiring trach and G-tube management. The schedule fluctuates between Sunday to Friday or Monday to Friday, depending on the familys needs. Schedule:

50 hours per week

Fluctuating between Sunday-Friday or Monday-Friday

Night shifts:8:00 PM – 6:00 AM

Responsibilities:

Provide high-quality nursing care, including trach and G-tube management, in a home care setting.

Monitor and document patient conditions, ensuring timely communication with the healthcare team.

Administer medications and perform routine nursing procedures.

Collaborate with the patient’s family and healthcare providers to ensure comprehensive care.

Responsibilities

Perform selected nursing procedures within the scope of LPN practice

Provide patient and family teaching according to plan outlined by the RN

Monitor patients vital signs and make specific measurements and observations

Monitor and record intake and output. Report significant changes immediately

Observe, assess and record patients physical and mental condition and reports any changes

Administer medications as instructed and as ordered by the physician and instructed by the RN and document the patients file accordingly

Attend and participate in multi-disciplinary patient case conferences, staff meetings and in-service education programs as assigned

Qualifications

Licensed Practical Nurse / LPN license in good standing

Previous private duty, home health, hospice, hospital or skilled nursing experience preferred

Active CPR certification

Familiarity with electronic health records

Strong problem solving and critical thinking skills

Reliable transportation, valid drivers license and current automobile insurance
