The LSW will assist us in maintaining our distinctive level of care by collaborating with residents/patients and their families to develop plans of care, monitor progress and ensure success.

The ideal candidate will:

Responsibilities include:

Collaborate with the multidisciplinary health care team

Interview residents/patients/representatives and assess their current condition, needs, strengths and weaknesses

Adhere to and follow the Social Work Code of Ethics and Standards of Practice

Develop and implement resident/patient-centered plans of care

Monitor and evaluate clients progress and modify treatment plans accordingly

Maintain accurate records and report on clients status

Refer clients to community services and assist with coordination of needs to ensure successful discharge plans

Qualifications:

Must hold a current MN license from the Board of Social Work

Social perceptiveness and empathy

Ability to build and maintain professional helping relationships

Ability to relate and communicate with diverse population and groups

Resilience, along with the ability to assess situations and problem-solve for therapeutic outcomes

Corporate Perks Program

This is a full-time position, working primarily daytime between 8:00 a.m. and 5:00 p.m. It includes a rotation of every fourth weekend and on-call holidays.
