Job Title: Coordinator – Integrated Transitional Care

Location: 10801 Sixth ST STE 120 Rancho Cucamonga California 91730 United States (Hybrid)

Pay Rate: $23.98/hr

Duration: 6 Months on W2

Job Summary:

The coordinator – Integrated Transitional Care will be responsible for processing authorization requests, verifying eligibility, and obtaining additional information as requested by the Integrated Transitional Care Nurse. The role involves managing non-clinical functions, collaborating with inter-departments, and ensuring timely and accurate support for member needs.

Key Responsibilities:

Process authorization requests promptly and accurately.

Verify member eligibility and obtain additional required information.

Support the clinical team by completing non-clinical functions as needed.

Collaborate with various departments to meet member needs.

Utilize knowledge of ICD 10 and CPT coding to ensure correct data entry.

Maintain member records in Electronic Medical Records (EMR) systems.

Provide customer service support, handling inquiries from members/clients.

Assist with managing electronic faxes and data entry tasks.

Uphold IEHP’s Quality Program goals, including HEDIS, CAHPS, and NCQA Accreditation standards.

Qualifications:

Minimum 2 years of experience as a Data Entry Specialist .

High School Diploma or GED required.

Strong knowledge of ICD 10, CPT codes , and medical terminology .

Experience in Managed Care or physician offices preferred.

Proficient in computer applications , including word processing, databases, and spreadsheets.

Excellent customer service skills.

Valid California Drivers License required.
