Eliot’s Behavioral Health Division provides an integrated and comprehensive network of services to adults with psychiatric disabilities that are evidence based, provide rehabilitation & hope to those in services.
Eliot is currently seeking an enthusiastic Assistant Team Leader to help oversee the multidisciplinary ACCS team. The ideal candidate will work alongside the Team Leader to ensure that recovery services, vocational services & clinical supports are included in the treatment plan developed for each individual served.
Responsibilities:
Assist with overseeing of all administrative and clinical functions of team and oversight of services for all Persons. Monitor overall service delivery and utilization of supports for up to 125 Persons.
Assist in the oversight and participate in the development of Treatment Plans (TP) for the Team and Persons Served.
Ensure that programs/teams are coordinating and overseeing all case management related services for Persons in programs. This includes assisting Persons with managing income, insurance, other entitlements, accessing other services and community resources and scheduling/coordination of all medical and psychiatric appointments.
Ensure that services are culturally competent, strengths based, recovery based, focused on rehabilitation and are inclusive of Person preferences. Ensure that the Person is at the center of all care and that all interventions are developed and provided in a manner consistent with Person preferences.
Ensure that recovery services, vocational services and clinical supports are infused in all aspects of care and intervention.
Prepare Persons and Teams for transition of vocational services to employment support services including but limited to Massachusetts Rehabilitation Commission.
Oversee the completion and direction of clinical assessments related to rehab option and assessments related to risk and crisis management. Ensure that assessments are inclusive of the Persons preferences, are strength based and culturally competent. Ensure that intakes are completed by clinical supervisors in timely manner and that risk is appropriately assessed.
Provide direct clinical interventions and adapt the individuals treatment plans as needed.
Provide on-call backup clinical support to case management staff on a rotating basis.
Schedule: Monday-Friday 8am-4pm
Salary: Starting at $85,000 annual salary
