The RN Case Manager Assistant provides comprehensive case management for patients at skilled nursing facilities. Responsibilities include coordination with SNF team members, nursing, rehab, discharge planning, dietary, admission, DON, administrator, and physicians. The role includes telephonic case management for the Special Needs Department if needed. The assistant conducts medical necessity screening and collaborates with the interdisciplinary team to provide care coordination under the direction of a Registered Nurse. This position ensures compliance with evidence-based practices and regulatory requirements. Key responsibilities include accurate medical necessity screening, care coordination, implementation of transition plans, communication with the interdisciplinary team, management of concurrent disputes, and clear documentation in electronic systems.
Hard Skills

This is a fully on-site position in Anaheim, California.
Work Environment
Monday to Friday with rotating weekends (every other weekend). Start time 07:00AM to end time 3:30PM.
