We are seeking an Advanced Practice Provider to join one of the nation’s most integrated healthcare systems, with over 1,400 providers in Indianapolis, Indiana.

Highlights

Community Physician Network seeks an Advanced Practice Provider to join our Palliative Care team in Indianapolis, IN. The most comprehensive palliative care program of its kind in Indiana, our team is committed to helping patients and families understand where they are on their medical journey and providing the physical and emotional comfort they need.

Flexible FTE (can be 0.6 – 1.0 FTE)

Weekend coverage to include a Friday/Saturday/Sunday OR Saturday/Sunday Monday schedule

This provider will work autonomously and collaboratively with on-call Nurse Practitioners and Palliative Care physicians in our network hospitals to deliver palliative care services to patients.

Focus on the comfort measures of patients, end of life care, symptom management, as well as assisting with consults and new weekend admissions.

Flexibility to travel between hospitals in Indianapolis

Qualifications

Ideal candidates will:

Possess 1-3 years’ Experience as a licensed Nurse Practitioner in Palliative Care (strongly preferred)

Be comfortable assessing patients independently

Be compassionate caregivers with excellent organizational, communication, and critical thinking skills

Possess highly-developed interpersonal and group dynamic skills, as well as proven skills as a consultant/educator to friends, families, and loved ones

Licensure & Certifications

Educational background consistent with requirements for licensure and certification within the state of Indiana
