This Social Worker Systemic Family Therapy (SFT)/Collaborative Intensive Bridging Services (CIBS)/Intensive In Home, a hybrid position. The CIBS/SFT/Intensive In Home social workers will provide intensive in-home clinical services with youth and their families. Youth/families referred for SFT/CIBS will be served in accordance with the CIBS/SFT model of service delivery. This includes individual therapy with the youth, family therapy with the entire family, and parent coaching and guidance with the parent(s)/legal guardian(s). CIBS/SFT social workers participate in weekly supervision and monthly team consultation with an outside consultant trained in the CIBS/SFT model. In some CIBS situations, youth participate in a 30 day out of home placement in a Residential Treatment Facility. The CIBS therapist continues to provide clinical services during this placement and partners with the CIBS Coordinator and the Ongoing CMH Case Manager. Duties & Responsibilities These examples do not include all possible tasks in this work and do not limit the assignment of related tasks in any position of this classification. Regular attendance according to the positions management approved work schedule is required for all positions. 1. Provide intensive in-home clinical services to youth and their families. For youth referred to SFT/CIBS clinical services will be provided according to the CIBS/SFT model of service delivery. 2. Develop and implement all or part of an individualized treatment plan, defining initial and ongoing measurable goals and objectives. Complete SFT/CIBS/Intensive In Home required paperwork. 3. Participate in SFT/CIBS required supervision and consultation. 4. Provide, arrange, and coordinate direct services and advocacy to clients and families. 5. Facilitate, integrate, and mobilize community resources on behalf of the client. 6. Closure of CIBS/SFT/Intensive In Home activity with client based on resolution of established goals and/or successful integration to community resources. Terminate activity with client when client requests closure. ESSENTIAL JOB FUNCTIONS: Duties 1-6 are essential functions. Knowledge, Skills & Abilities and Work Environment Knowledge of child and human development, family dynamics and systems, including crisis management and cultural competency. Knowledge of Trauma Informed clinical practices. Openness to learn and receive feedback. Knowledge of community resources and funding sources. Knowledge of medical diagnoses, medications, and basic medical terminology, as well as medical insurance processes and procedures. Knowledge of statutes, rules, and policies pertaining to social work and the ability to research the same. Interviewing, negotiation, and listening skills. Ability to provide clear, direct expectations and feedback with youth and families when appropriate. Ability to communicate effectively both orally and in writing. Demonstrated knowledge and ability to assist youth and families in implementing sy temic family interventions. Demonstrates an understanding of and respect for the diversity of customers, co-workers, and supervisors; including individual with a disability or whose first language may be other than English. WORK ENVIRONMENT: Work is performed in a hybrid model. Work is performed in a county office building, in community locations, on client home visits, and via telehealth. Some travel is required and reimbursed. Lifting requirement of up to 20 pounds on an occasional basis. Exposure to inadequate lighting may occur on client home visits. Equipment used may include, but is not limited to, computer, printer, telephone, cell phone, pager, audio/visual equipment, and copy and fax machines.
