Job Description
Provides counseling services to bereaved family members of hospice patients and the general community.
Job Responsibility
1.Assesses bereaved clients and provides program orientation.
2.Collaborates with healthcare team members to develop and implement client plan of care.
3.Conducts individual, group and family counseling sessions.
4.Serves as a liaison to referral sources to facilitate continuity of care.

Job Qualification
1.Masters Degree required.
2.Current License to practice as a Master Social Worker, or Current License to practice as a Clinical Social Worker in New York State, or Current License to practice as a Mental Health Counselor in New York State required, plus specialized certifications as needed.
3.1-3 years of technical experience, required.

The salary range for this position is $58,500-$75,012/year
