Social Worker / Counselor

The Unlicensed Social Worker/Counselor/Mental Health Clinician provides counseling to patient’s in a variety of settings including medical facilities, hospitals, clinics, learning centers and other organizations that are in need of assistance.
Essential Duties and Responsibilities:
Provides necessary counseling as required for the patient/client
Assesses the patient/clients’ psychosocial needs, and evaluate the progress with the care team and/or supervisor
Collaborates with all disciplines to plan and evaluate goals for each patient/client
Focuses on assisting patients/clients through the discharge process of both in and out-patient treatment facilities as applicable
Utilizes a variety of therapy techniques and treatment plans
Facilitates patient progress and referrals with other agencies
Adheres to all Amergis and Facility policies and procedures
Observes and reports patient/client progress per facility protocol
Performs other duties as assigned/necessary
Minimum Requirements:
Licensing/Certification according to state/facility/contract requirements
The Counselor may possess an Associate’s Degree, Bachelor’s Degree or Master’s Degree in Psychology, Sociology, Social Work, Counseling, Nursing or other Human Development Major
Complies with all relevant professional standards of practice
Participation and completion of Amergis Competency program when applicable
Current CPR if applicable
TB questionnaire, PPD or chest x-ray if applicable
Current Health certificate (per contract or state regulation)
Must meet all federal, state and local requirements
Successful completion of new hire training as applicable to job site
Understand patient confidentiality and HIPAA requirements
Ability to effectively elicit/provide information to and from appropriate individuals (including, but not limited to, supervisors, co-workers, clients) via strong communication skills; proficiency in the English language is required
Computer proficiency required
Must be at least 18 years of age
