Position: Case Manager Part time
The Case Manager is responsible for the delivery of care coordination services to a population with history of substance abuse. The Case Manager serves as a member of the interdisciplinary treatment team.
Job Scope:
• Provide clinical case management to clients who reside in the residence.
• Counseling and support to enhance clients’ capacity to benefit from treatment regimen.
• Crisis intervention to assist clients in dealing with sanctions, barriers to employment and self-sufficiency.
• Engage in community outreach to locate appropriate clients & assist through enrollment process.
• Assessment and support services to facilitate access to specialized services e.g. Mental, Health, Social Security Disability, Veteran’s Resources, etc.
• Assist in facilitating access to job training, job search activities, placement and stabilization.
• Advocacy and report of client activity to community-based organizations and clients’ referral / mandating sources.
• Adherence to agency protocols for maintenance and security of agency equipment.
• Participate in weekly case conference.
• Complete client-centered treatment plans.
• Meet with each assigned client weekly to identify progress towards case management goals.
• Provide transportation to required appointments when necessary
• Observe medication self-administration under supervision
Qualifications:
CASAC-T; CASAC, or higher preferred.
1 year of experience working with clients with substance abuse disorders.
1 year of experience working with adolescents with co-occurring and mental health problems.
1 year of experience working in an OASAS-funded facility a plus
Commitment to providing quality services and outcome-driven performance measures.
Excellent organizational management skills with the ability to coach staff, manage, and develop high-performance clinicians, set and achieve program objectives.
Strong networking and public relations skills with the ability to engage a wide range of stakeholders, populations, and cultures.
An action-oriented, adaptable, and innovative approach to program planning.
Ability to work effectively in collaboration with diverse groups of people.
Supervised by: Assistant Program Director in collaboration with the Program Director
Position Status: This is a part-time position. The Work Schedule for this part-time position is Monday 10am – 4pm, Wednesday 10am – 4pm, Friday 4pm – 10pm
For this long island, NY position the base rate is $22 – $25/ hour
Work Environment
This job operates in a professional office environment and outside in the field. This role routinely uses standard office equipment such as computers, phones, photocopiers, filing cabinets and fax machines. Must be able to travel among assigned sites and locations as needed.
