Minimum Qualifications A bachelor’s or higher-level degree in a human services field a degree in a human services field includes the professional disciplines of audiology, community mental health, nursing, nutrition, occupational therapy, physical therapy, psychology, rehabilitation counseling, social work, speech/language pathology, and therapeutic recreation (including concentrations in art, creative arts, dance, drama, music, or physical education.) Also included are related academic disciplines associated with the study of human behavior (e.g., gerontology, human services, sociology, speech communication); or human skills development (e.g., counseling, education, human development.) Examples of non-qualifying degrees include but are not limited to: criminal justice, political science, liberal arts, and business.

Duties Description • Conduct monthly visits to the Family Care homes and document the visits on the OPWDD Form 239.• Ensure that all regulatory and environmental requirements related to Family Care homes are met including the safety of the home, health care, and safeguarding needs of individuals.• Assess the appropriateness of individuals’ activities based on the Residential Habilitation Staff Action Plan.• Schedule and attend annual and semi-annual Life Plan meetings, and special meetings as needed.• Develop/review/revise Staff Action Plans and Safeguard Summary Plans.• Ensure completion of monthly progress notes.• Review Personal Allowance ledgers monthly to ensure the appropriateness of expenditures.• Assist with Family Care provider recertification process.• Process payments and reimbursements to the Family Care Providers.• Comply with policies/procedures for Family Care.• Prepare Residential Habilitation documentation for audits and claims reviews.• Establish working relationships with Care Managers, clinicians, day/work programs to ensure Life Plan meetings are conducted and services are coordinated. • Additional responsibilities as assigned by supervisor.• Travel required.
