RN, Case Management, PRN
Northwest Florida Community Hospital
Provides case management services to patients, assists with developing discharge and post-hospital care plans, acts as liaison between medical/nursing staffs, patients, families, community agencies, etc., assists social worker(s) with specific problems, and assists with utilization review functions as needed.
Education: RN Licensure required
Experience : At least one year hospital clinical experience required. Experience in Social Services/Discharge Planning/UR preferred.
Licenses, Certifications and/or Registrations:
State of Florida Board of Nursing RN Licensure; American Heart Association BLS Healthcare provider certification.
